Job Description

In this community nursing role, you as a Registered Nurse (RN) will have scheduled visits within the community to care for patients in a school settingYou will work with school agedpatientsdiffering complexity of care needs and play a key role in helping patients remain comfortable for as long as possible. Whatyou’lldo:

  • Participate in scheduled visits in the community to look after patient needs within your scope ofpractice

  • Provide ethical and safe nursingcare

  • Useappropriate assessmenttools/techniques to assess patients’ individualneeds

  • Execute and continuouslymonitorcare plans and adjust asrequired

  • Engage patients using a collaborative approach by building rapport andtrust

  • Maintain and update professional patient records related to patient encounters andplans

What you bring:

  • Registration with the CNO as a Registered Nurse in good standing 

  • Valid driver’s license and access to a vehicle isrequired

  • Experience in delivering community care is anasset

  • Reliability and flexibility  

  • Willingness to travel to and from patient residences within a designatedarea
