Job Duties:

Provide Targeted Case Management to children with special health care needs and their families that include screening program applications to determine initial and ongoing medical eligibility; conducting a needs assessment; and developing service plans that bridge clinical care and community resources to improve health outcomes for children with special health care needs.

The Case Manager will maintain a caseload, and utilize the service plan as a foundation to coordinate services each child’s needs in the community amongst other providers that include the following, but not limited to: arranging medical services according to program policy; making in-person visits with clients as deemed necessary and/or required by policy; communicate with agencies, providers and individuals to develop regional infrastructure for children with special health care needs; participate in regional planning teams utilizing knowledge of pediatric nursing; attend specialty clinics as assigned to assist families with medical needs and provide other agency resources; etc.

Maintain detailed and accurate case notes for each client and document all services and contact with families and providers in an electronic case management database system for the child.

Attend continuing education and in-service training sessions to stay informed of Federal, State, and agency policies and procedures regarding pediatric nursing and case management to children with special health care needs and their families.

In-State Travel is required to attend Outreach Clinics, statewide meetings, and conduct In-Person/Home Visits.

Salary Range: $75,245 - $111,363


Registered Nurses (RNs) provide direct patient care in a variety of healthcare settings, including hospitals, clinics, and community health programs. RNs assess patient health, develop care plans, administer treatments, and collaborate with other healthcare professionals to deliver high-quality care.

Primary Responsibilities


Assess patient health, monitor vital signs, and document patient progress.
Administer prescribed medications and treatments in accordance with medical orders.
Develop, implement, and update patient care plans.
Educate patients and families on health conditions and wellness practices.
Participate in multidisciplinary care teams to plan and implement care strategies.
Maintain accurate patient records and comply with health regulations.

Knowledge and Skills


Knowledge of nursing principles, practices, and procedures.
Ability to perform physical assessments and recognize changes in patient condition.
Proficiency in administering medications and treatments safely.
Strong communication skills for patient interaction and team collaboration.
Ability to prioritize tasks and manage time effectively in a fast-paced environment.

Minimum Qualifications

Current Registered Nurse (RN) licensure in Arkansas.

Satisfaction of the minimum qualifications, including years of experience and service, does not entitle employees to automatic progression within the job series. Promotion to the next classification level is at the discretion of the department and the Office of Personnel Management, taking into consideration the employee’s demonstrated skills, competencies, performance, workload responsibilities, and organizational needs.
