Job Description

UNIT PROFILE:
The Home Dialysis Program provides comprehensive and collaborative education, assessment, and ongoing support for patients and families/support persons who perform hemodialysis or peritoneal dialysis in the home setting. Nurses in this program promote patient independence through consistent, evidence‑informed teaching and clinical support whether during clinic follow up appointments, at patient’s homes or when providing dialysis training sessions. Care is provided in alignment with unit policies, provincial regulatory standards, and best‑practice guidelines for home‑based renal replacement therapy.


The Registered Nurse (RN) is a regulated health care professional who practices in accordance with the Standards and Guidelines of the College of Nurses of Ontario, as well as the mission, vision, and values of St. Joseph’s Healthcare Hamilton. The RN delivers patient‑centred, evidence‑informed care to support health promotion, disease management, and the treatment of chronic kidney disease through supportive, preventative, therapeutic, rehabilitative, and palliative means.

The RN collaborates with the interprofessional team and assumes a leadership role in providing a full range of care to patients with complex needs, and variable outcomes. Within the Home Dialysis Program, the RN provides specialized assessment, training, and ongoing support for patients performing home hemodialysis and peritoneal dialysis, ensuring safe, effective, and independent/supportive treatment in the home environment.

QUALIFICATIONS:

  • Current registration with the College of Nurses of Ontario required
  • Current BCLS/HCP certification required
  • Minimum 2 years of recent clinical dialysis experience required (Hemodialysis and/or Peritoneal Dialysis)
  • CNeph(C) certification preferred
  • Ability to conduct meetings with patients and families in their home as required
  • Membership in RNAO, CNA, or other professional associations preferred
  • Gentle Persuasive Approach training preferred
  • Completion of continuing education in patient/adult education and physical assessment preferred
  • Ability and commitment to supporting patients in choosing and maintaining their preferred dialysis modality
  • Demonstrated knowledge, sensitivity, and expertise in the care of seniors and their families
  • Demonstrated ability to provide comprehensive care to dialysis patients with complex needs, and less predictable outcomes
  • Excellent written and verbal communication skills with patients and families and the inter disciplinary team
  • Demonstrated ability to work collaboratively within an interdisciplinary team
  • Strong organizational skills with the ability to set priorities based on patient and unit needs
  • Commitment to ongoing learning, professional development, and continuous quality improvement
  • Basic computer proficiency (MS Word, incident reporting systems, data entry, patient information systems)
  • Excellent interpersonal and conflict-resolution skills
  • Strong leadership, critical thinking, problem‑solving, team‑building, and interpersonal skills
  • Competence in identifying and responding to treatment associated complications
  • Reliable attendance and work record
