Principal Responsibilities and Duties:
● Provides coordinated care to adult and pediatric patients by collaborating with patients and interdisciplinary care team members to ensure comprehensive, patient-centered outcomes.
● Conducts comprehensive health assessments, records medical histories, completes medication reconciliations, evaluates patient conditions, and contributes to individualized care and treatment decisions.
● Works effectively in both clinic and urgent care settings, utilizing critical thinking skills to make sound clinical judgments and prioritize patient needs in a fast-paced environment.
● Performs nurse visits, providing clinical assessments, patient education, and nursing interventions following established clinical protocols and scope of practice.
● Educates patients and their families about health conditions, treatment plans, and preventive care, proactively addressing gaps in care.
● Provides team-based nursing care within assigned pods, collaborating with providers and support staff to ensure coordinated and efficient patient care.
● Administers prescribed medications, vaccines, and treatments following approved clinical protocols and standards of nursing practice. Ensures accurate and timely documentation of all administrations in the electronic medical record.
● Collaborates with physicians, other nurses, and healthcare professionals to coordinate patient care and ensure smooth transitions.
● Responds promptly to all patient-related telephone calls and in-basket messages, ensuring timely communication and continuity of care.
● Participates in health center outreach initiatives and community activities to promote public health and strengthen community engagement.
●Maintains strict confidentiality of all patient and health center information in accordance with HIPAA and organizational policies.
● Other duties as assigned.
Required Skills:
● Excellent clinical assessment and critical thinking skills, with a solid understanding of disease processes and age-specific development.
● Demonstrated ability to deliver high-quality nursing care in collaboration with physicians and support staff.
● Culturally sensitive, with a strong interest in serving diverse patient populations
● Ability to establish and maintain effective working relationships with other employees, individuals seeking assistance, representatives of outside agencies, and the general public.
● Proficient in electronic health record (EHR) systems and familiar with outpatient and ambulatory care best practices.
● Excellent verbal, written, and interpersonal communication skills.

● Bilingual preferred (Spanish, Russian, or Vietnamese).
Qualifications:
● Current and unrestricted RN license in Massachusetts
● Current BLS CPR certification
● Minimum of one year of nursing experience.
Required to cover extended clinic hours, including evenings and weekends, on a rotating basis at any
Caring Health Center location.
Working Conditions:
● Work is performed in a busy outpatient clinic environment with direct patient contact.
● Requires frequent standing, walking, bending, and lifting.
● Involves the use of computers, phones, and other standard medical office equipment.
● Must be able to respond quickly in urgent or emergent situations.
