The Clinical Triage RN directs patients by a patient centered approach to the appropriate level of care based on the severity of their clinical need. The severity of the patient's needs are assessed using protocols, algorithms and clinical judgement that can provide patient disposition to urgent care, emergency care, clinic provider appointments, and /or other services. The Clinical Triage RN will support communication to the patient, family and support systems as appropriate. Exhibits optimal partnership with clinical practices, contact center teams and other community services/resources. Facilitates information through a propriate communication and documentation. The Clinical Triage RN role contributes to guiding patients to the most appropriate site of care based upon their stated care needs, and the interruption of frequent acute hospital or emergency department use patterns.

Schedule Full-time, 37.5 hrs., 8:30 AM – 5PM. Holiday Recess Period may be required.

Required Skills and Abilities

1. Licensure as a Registered Nurse in the State of Connecticut (or eligibility for Connecticut licensure) and current CPR certification.

2. Superior communication, interpersonal, assessment, listening, and problem-solving skills with demonstrated cultural competency and the ability to advocate for optimal patient outcomes.

3. Ability to multitask and meet performance standards in a fast-paced, high-stress environment while maintaining sound clinical judgment.

4. Excellent written, verbal, and organizational skills, with the ability to handle confidential and sensitive patient information professionally and in compliance with all HIPAA regulations.

5. Demonstrated commitment to teamwork, reliability, and professionalism, including a proven record of exemplary attendance, punctuality, dependability, and a caring, motivated approach to patient care and team collaboration.


Preferred Skills and Abilities
Prior experience in ambulatory triage or care coordination and chronic diseases. Prior experience with EPIC Ambulatory EMR. Prior experience using ACD lines and tracking boards.

Principal Responsibilities

1. Performs physical assessments of patients, including triage duties and advises on treatment or course of action. 2. Identifies and evaluates nursing services problems and develops and implements appropriate programs and services, including assessing staffing needs and arranging coverage as needed. 3. Develops, implements, and manages nursing care plans in collaboration with physicians. 4. Instructs patients and families in proper treatment and advises patients on health maintenance and disease prevention. 5. Provides leadership, guidance, direction to clinical staff, and collaborates with nursing at all levels. 6. Determines standards for quality assurance and infection control procedures; trains staff in universal and isolation precautions in the area of infection control. 7. Performs diagnostic and therapeutic measures in collaboration with physicians. 8. Evaluates the quality and effectiveness of nursing practice and nursing services. 9. Records patients’ medical histories and symptoms, performs diagnostic tests and analyzes results, operates medical machinery, administers treatment and medications, and coordinates with patient follow-up and rehabilitation; assures all procedures for maintaining patient confidentiality are maintained per HIPAA guidelines. 10. May perform other duties as assigned. Required Education and Experience
Bachelor’s Degree in Nursing and two years’ experience or an equivalent combination of education and experience.
