Our Adult Diabetes Services team supports over 15,000 patients annually through the Diabetes Management Centre (DMC) and Centre for Complex Diabetes Care (CCDC). We are seeking an experienced and passionate Registered Dietitian to join our highly collaborative interdisciplinary team.

About the Centre for Complex Diabetes Care (CCDC)

The Centre for Complex Diabetes Care (CCDC) provides specialized, interdisciplinary care for adults living with diabetes and multiple co-existing health conditions. Designed to support patients whose needs extend beyond conventional diabetes management, the program brings together a team of healthcare professionals who work collaboratively to deliver coordinated, patient-centred care.

Through a short-term case management model, the CCDC supports individuals living with Type 1 and Type 2 diabetes who may also be managing conditions such as cardiovascular disease, kidney disease, mental health challenges, and other complex medical concerns. The team works closely with each patient to identify barriers to self-management, optimize treatment plans, and improve overall health outcomes.

As a Registered Dietitian within the Centre for Complex Diabetes Care, you will provide specialized nutrition assessment, counselling, and diabetes education to individuals living with Type 1 and Type 2 diabetes, often accompanied by renal, cardiovascular, mental health and other complex medical conditions.

Using evidence-based nutrition therapy, motivational interviewing, and adult learning principles, you will support patients in developing sustainable self-management strategies while contributing to integrated care planning within an interdisciplinary team.

This position is based onsite at the Centre for Complex Diabetes Care due to the highly collaborative, team-based case management model and the complex needs of the patient population

Key Responsibilities:

  • Conduct comprehensive nutrition assessments and develop individualized care plans

  • Provide one-on-one and group diabetes education using evidence-based approaches

  • Support patients with complex diabetes and multiple co-morbidities

  • Participate in interdisciplinary case conferencing and discharge planning

  • Develop and update patient education resources

  • Collaborate with community partners and health care providers

  • Participate in quality improvement, program planning, and evaluation activities

  • Maintain accurate and timely documentation within the electronic medical record

Key Qualifications:

  • Degree in Human Nutrition and Dietetics combined with an accredited Dietetic Internship program
  • Member in good standing with the College of Dietitians of Ontario
  • Certified Diabetes Educator an asset
  • 3-5 years experience providing nutrition counselling and diabetes education in a healthcare setting

  • Strong knowledge of diabetes management and nutrition therapy across the adult lifespan

  • Experience working with complex chronic disease populations

  • Knowledge of motivational interviewing and behaviour change strategies

  • Ability to work independently while contributing effectively within an interdisciplinary team

  • Proficiency with electronic medical record systems and computer applications

  • Positive work and attendance record essential
