Job Description

New!We are opening a new Portland Kidney Center in 2026, and Northwest Kidney Centers is hiring a Registered Dietitian to join our Nutrition Team in Portland, OR (On-site).

Purpose
Clinical Registered Dietitians provide nutrition care to patients through assessment of nutritional status, development of individualized nutrition care plans, implementation of appropriate interventions, and ongoing monitoring of patient progress in accordance with provider orders, regulatory requirements, NKC policies, and department guidelines.

The Registered Dietitian (RD) is a member of the interdisciplinary team (IDT) and is responsible for completing comprehensive nutrition assessments and developing individualized plans of care that meet patient needs and Conditions of Coverage.

The RD participates in Quality Assessment and Performance Improvement (QAPI) activities and collaborates with the interdisciplinary team to support improved patient outcomes. Responsibilities also include providing nutrition education and counseling to patients, caregivers, and members of the care team, as well as participation in NKC-sponsored education and outreach activities.

Shift: Part-time (0.2FTE), Monday - Friday

Required qualifications:

  • Bachelor’s degree required.
  • Must be a Registered Dietitian credentialed by the Commission on Dietetic Registration (CDR).
  • Approximately one (1) year of professional experience in clinical nutrition or a related setting preferred.
  • Must be certified or licensed and registered in the state where services are provided, in accordance with applicable state requirements.
  • Must have a valid driver’s license.

Essential Functions:

1. Patient Assessment and Care Planning:

  • Independently analyze and assess patient nutritional, medical, and social information from medical records, patient interviews, laboratory values, and interdisciplinary input.
  • Develop comprehensive nutrition assessments and individualized plans of care based on identified patient needs.
  • Collaborate with the interdisciplinary team (IDT) to support development and ongoing management of patient care plans

2. Care Implementation and Monitoring

  • Implement nutrition interventions in accordance with provider orders, clinical guidelines, and regulatory requirements.
  • patient response to interventions and adjust care plans as appropriate.
  • Provide clinical input to the care team to support patient outcomes, including review of laboratory data and trends.

3. Documentation and Compliance

  • Document all assessments, plans of care, interventions, and patient interactions in the electronic health record in a timely and accurate manner.
  • Ensure compliance with NKC policies, regulatory requirements, and Conditions of Coverage.

4Patient and Caregiver Education

  • Provide individualized and group nutrition education to patients, caregivers, and staff as appropriate.
  • Develop and utilize educational materials to support patient understanding and engagement in care.
  • Support patient engagement in appropriate health and wellness activities.

5. Quality Improvement and Team Participation

  • Participate in Quality Assessment and Performance Improvement (QAPI) activities, including evaluating trends and contributing to improvement initiatives.
  • Participate in unit meetings, huddles, and interdisciplinary team discussions to support patient care and operational goals.

6 Coordination and External Collaboration

  • Coordinate care with Registered Dietitians, Social Workers, Nurse Care Managers, nursing staff, physicians, and other members of the care team.
  • Communicate with external providers, organizations, and community resources as needed to support patient care.
