Compensation: $60,000 - $65,000 / year


Schedule: 40 hours minimum per week | 8:00 am - 4:30 pm | Monday - Friday

What You'll Do In This Role:
Make nutrition a powerful tool for healthier, more independent living. As a Registered Dietitian with PACE, you’ll play a vital role on the interdisciplinary team, providing comprehensive nutrition assessments and developing personalized care plans that support each participant’s health and quality of life. Collaborating closely with medical, nursing, rehabilitation, and social services teams, you’ll address chronic conditions, functional challenges, and age-related nutrition needs—helping older adults thrive while maintaining dignity and independence.

Key Responsibilities

Clinical Nutrition Care

  • Conduct comprehensive nutrition assessments upon enrollment, semi and annually, and with any significant change in participant condition.
  • Develop, implement, and monitor individualized nutrition care plans based on medical history, functional status, cultural preferences, and psychosocial needs.
  • Provide medical nutrition therapy (MNT) for chronic diseases such as diabetes, cardiovascular disease, renal disease, malnutrition, and dysphagia.
  • Monitor weight trends, laboratory values, oral intake, and nutritional risk indicators.
  • Recommend therapeutic diets, oral nutrition supplements, and texture‑modified diets as clinically indicated.

Interdisciplinary Team (IDT) Participation

  • Actively participate in IDT meetings, care planning, and case conferences.
  • Collaborate with providers, nurses, social workers, behavioral health clinicians, and home care staff to coordinate holistic care.
  • Provide nutrition‑related input for participant goals, risk mitigation, and care transitions.

Nutrition Education

  • Provide individualized nutrition education and counseling to participants and caregivers.
  • Support behavior change through culturally sensitive, participant‑centered education.
  • Educate staff on nutrition‑related topics relevant to geriatric care.
  • Provides nutrition education to staff and the community in wellness groups and other outreach opportunities.

Home and Community-Based Care

  • Conduct home visits as needed to assess food access, safety, and ability to prepare meals.
  • Coordinate with home care teams to address nutrition‑related barriers in the community setting.
  • Organize home delivered meals and supplements as required.

Documentation and Compliance

  • Maintain accurate, timely, and compliant documentation in the electronic health record (EHR).
  • Ensure compliance with CMS PACE regulations, state and federal guidelines, and organizational policies.
  • Participate in quality improvement initiatives and audits as assigned.

Skills, Knowledge & Expertise

  • Master’s degree in Nutrition, Dietetics, or related field.
  • Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential through the Commission on Dietetic Registration.
  • Minimum of one (1) year working with the frail and elderly.
  • Certified Diabetes Educator (CDE) desired.
  • ServSafe Food Protection Manager certification desired.
  • Minimum of 1–2 years of clinical nutrition experience (geriatrics, long‑term care, or community health preferred).
