Vancouver Coastal Health (VCH) is seeking an engaging and collaborative clinical staff leader for the role of Regional Program Medical Director, Primary Care, Home and Community Care who fosters trust, credibility and strong working relationships across the primary care system.

Reporting to the Vice President, Medicine and Academic Affairs (VPMAA), the Regional Program Medical Director, Primary Care, Home and Community Care operates under the strategic direction of the Executive Sponsor and works in a dyad partnership with the Executive Director to provide senior medical leadership for advancing integrated planning, alignment, and delivery across primary care, home health, home support and community care including transition support. The time commitment for this role is expected to be 0.6 FTE in alignment with the scope and responsibilities of the role.

The Regional Program Medical Director co-develops and advances regional strategies aligned with Ministry of Health direction and VCH priorities, supporting an integrated, population-based approach to care across the continuum. This includes strengthening access, quality, continuity, and coordination across community, residential, and acute care interfaces.

The role builds and sustains strong partnerships with Communities of Care, Divisions of Family Practice, medical and operational leaders, academic partners, and community providers. Working closely with Indigenous Health leadership, the role advances culturally safe, anti-racist, and equity-oriented care. While regional planning and alignment sit within the program, operational delivery and performance accountability remain with Community of Care leadership.

This role operates within a model of shared accountability and partnership, recognizing the distributed nature of medical staff and Credentialed Medical Provider (CMP) governance and the intersection with operational, academic, and corporate functions. Key Accountabilities

With direction from the Executive Sponsor and in partnership with the Executive Director, this role is accountable for the following:

Strategic Medical Leadership and Program Planning

• Provides senior medical leadership for the integrated planning, development, and evaluation of Primary Care, Home Health, Home Support, and Community Care including Transition Support across the region.
• Co-develops and advances regional strategies, priorities, and multi-year plans aligned with Ministry of Health direction, VCH priorities, and population health needs.
• Integrates medical leadership into service design, policy development, and system planning across the continuum.
• Identifies emerging risks, pressures, and opportunities and advises senior leadership accordingly.

Dyad Leadership and System Alignment

• Works in a formal dyad partnership with the Executive Director with shared accountability for strategy, alignment, and outcomes.
• Aligns regional priorities with implementation across Communities of Care, supporting coordinated execution and consistency of approach.
• Builds and sustains alignment across medical, operational, and academic leadership, including Regional Department Heads and site/program leaders.
• Leads and/or co-chairs regional program structures (e.g., committees, working groups) to enable effective planning, decision-making, and communication.

Integrated Access, Equity, and Continuum of Care

• Advances equitable and timely access to integrated, person- and family-centred care across primary care, home health, home support, and community care including transition support.
• Strengthens coordination and transitions across the continuum, including acute, community, and residential care settings.
• Leads the development and implementation of regional approaches to complex care, frailty, chronic disease management, prevention and population health initiatives.
• Partners with Indigenous Health leadership to advance culturally safe, Indigenous specific anti-racism, and equity-oriented care and ensure alignment with Indigenous Health priorities.
• Engages patients, families, care partners, and communities to inform service planning and improvement.

Quality, Safety, and Clinical Performance

• Leads regional quality improvement initiatives and promotes consistent clinical standards and best practices across all services.
• Leads the development and monitoring of performance indicators to improve access, quality, patient outcomes, and experience.
• Supports review and follow-up of patient safety events, clinical risks, and system issues requiring medical leadership.
• Ensures program decisions and improvements are informed by data, evidence, and continuous learning.

Partnerships, Representation, and System Advancement

• Builds and sustains strategic and effective relationships with medical staff and CMP, Divisions of Family Practice, Primary Care Networks, community partners, and the Ministry of Health to support integrated service planning and delivery.
• Represents the regional programs internally and externally, including participation in regional and provincial initiatives and forums.
• Supports innovation, research, teaching, and the spread of leading practices across the continuum.
• Encourages adoption of new models of care, technology, and approaches that improve integration, access, and outcomes.
• Partners in the recruitment, engagement, and development of medical leaders within the program, as appropriate.

Qualifications

• A Doctor of Medicine Degree or Nurse Practitioner Degree and certification or licensure in good standing with the appropriate regulatory college.
• For a holder of a Doctor of Medicine Degree, eligibility for a faculty appointment with the University of British Columbia Faculty of Medicine or with the Simon Fraser University School of Medicine; for holder of a Nurse Practitioner Degree, eligibility for a faculty appointment with a School of Nursing at a recognized post-secondary institution in British Columbia.
• Eligibility for medical staff privileges or CMP at VCH.
• Minimum 5 years’ medical leadership experience in a complex health system, with experience in primary care, home health, community health services, or integrated community-based care.
• Demonstrated ability to lead system-level planning, service integration, and quality improvement initiatives across care settings.
• Proven ability to work effectively in a dyad leadership model and build strong relationships across medical staff and CMP, operational leaders, and partners.
