JOB SUMMARY:

The Regional Healthcare Manager (RHCM) is responsible for developing a network of referral sources, performing evaluations of prospective Gaylord Hospital patients, facilitating the admissions process and communicating Gaylord’s value proposition with healthcare organizations, referring healthcare practitioners, family members and insurance companies. Meets referral and admissions goals.

ESSENTIAL DUTIES AND RESPONSIBILITIES (Performance Appraisal Competencies): Include the following. Other duties may be assigned.

Evaluates and admits prospective patients referred to Gaylord Hospital

a)receives referral information electronically or via phone

b)pre-registers patient in Expanse, tasks benefits to the Admissions Intake Rep and performs on-site/remote pre- admission evaluation,

c)Uses Interqual, NALTH and Gaylord admissions criteria to clinically accept patients or discuss patients with other internal Gaylord staff to determine clinical appropriateness

d)consults with the Admissions Office regarding special needs of incoming patients to be communicated to the receiving Gaylord personnel. Needs include but are not limited to specialty beds, wound products, non-formulary medications, infection status.

e)communicates with hospital physicians, care managers, patient and family members regarding admissions process, providing feedback on the status of the referral, answering questions and obtaining any additional information, as indicated.

Manages account and/or territory

1.Establishes network of referral sources to increase referrals and admissions

2.Meets monthly and annual goals for referral generation and patient admission growth

a)conducts in-services and educational rounds to referral sources informing them of Gaylord Hospital services and admission criteria

b)performs ongoing needs assessments with Business Manager to identify referral sources’ needs for program development

c)distributes marketing information to referral sources

d)monitors referral trends

e)exhibits at pertinent conferences

f)Maintains Reptrax or Vendormate credentialing as required

Financial Stewardship of Gaylord Hospital

a.Monitoring for qualifying stay, interrupted stay, and coordination of benefits

b.Communicate out of pocket expenses to patient and family

c.Identify possible non-formulatory medications- clarify if there are alterntatives

d.Question planned non-urgent out-trips with acute care hospitals.

Actively participates in Admissions Department operations

a)Documents all data points in Expanse for department trending puropses including but not limited to acute care admissions date, referral date, appropriate program, reason for non-placement

b)attends and participates inAdmissions staff meetings

c)assists in orientation and training of new Regional Healthcare staff

d)complies with all Admissions policies and procedures

e)acquires and maintains skills and knowledge regarding Admissions through participation in relevant educational programs

f)serves on hospital-wide committees and projects

QUALIFICATIONS:

*ASN required; BSN or higher preferred

*Current CT Rn or LPN license required

*Valid CT Driver's license required

*4 or more years of RN or LPN experience in the healthcare field required, including 2 years in rehabilitation or insurance care management required

*Clinical assessment skills and knowledge of reimbursement issues impacting inpatient rehabilitation required

*Understanding and working knowledge of InterQual and NALTH criteria used for admissions preferred
