CatholicCare Central Queensland is seeking a highly organised and customer-focused Referral Officer to join our Aged Care team. In this role, you will be the first point of contact for new clients, ensuring referrals are managed efficiently and that every person experiences a welcoming and seamless journey into our services.

About the role:

The Referral Officer is responsible for the effective and efficient management of incoming referrals for aged care services across multiple sites and funding sources.

Key responsibilities

  • Review and assess referrals from various sources, including referral portals and the referral inbox, and assign them to the appropriate team or individual;
  • Contact clients to confirm receipt of referrals, schedule initial intake appointments, and provide clear information on the next steps in the intake process;
  • Set up preliminary profiles in the client management system, maintain up-to-date client information, and add case notes as needed;
  • Support a positive and responsive client onboarding experience by maintaining timely communication, following up prospective clients and identifying other CatholicCare services that may meet their needs; and
  • Collaborate closely with team members and other departments to ensure a smooth and efficient referral process

Qualifications and Experience:

Essential:

  • Experience in a referral or intake role within a healthcare or community services environment;
  • Experience working with client management systems and handling sensitive client information; and
  • Demonstrated ability to manage administrative tasks with attention to detail.

Desirable:

  • Certificate III in Community Services, Aged Care, or a related field, or equivalent work experience; and
  • Knowledge of aged care funding streams, experience with portals such as My Aged Care, and expertise in referral management processes

What you’ll bring

  • High level of verbal and written communication skills, with the ability to engage effectively with clients and team members;
  • High level of organisational skills to manage a high volume of referrals and administrative tasks efficiently;
  • High level of attention to detail to ensure accurate and up-to-date records in the client management system;
  • Sound level of experience in the use of applicable corporate systems and client management systems, including the Microsoft Office Suite; and
  • Sound level of Cultural competency, with the ability to provide services to clients from diverse backgrounds.

The successful candidate will also be required to demonstrate entitlement to work in Australia, and to disclose any pre-existing injury or disease which may be adversely affected by undertaking the inherent requirements of the position.

All offers of employment are subject to the satisfactory completion of relevant screening and background checks, including an Australian Federal Police check, an NDIS disability worker screening check, and attainment of a Blue Card for working with children, prior to commencement.
