At Unity Hospice, we bring peace of mind and comfort to patients and families when it matters most. We’re growing and looking for a Referral & Intake Nurse (LPN) to join our team and serve as a key connection point for patients, families, and referral partners across all Unity territories.
This role is ideal for someone who thrives in a fast-paced environment, enjoys coordination and communication, and is passionate about supporting patients at the very start of their hospice journey.
Work environment: Remote
Schedule: Part-time (3 days per week, 24 hours per week) 8:00 a - 4:00 p or 9:00 a - 5:00 p,, working every 4th weekend, 1-2 holidays per year.
