Job Description

Position Purpose:

Oversee the duties and responsibilities of Referral Clerks/Coordinators, as well as perform the hands-on duties as out-lined below.

The Referral Coordinator Lead will ensure that patients needing specialty services can access them in a timely and efficient manner. The lead will have ongoing interactions with the team members to discuss roles, responsibilities, communication, and patient hand-off, working together to provide and enhance the care provided to patients.

MAJOR POSITION RESPONSIBILITIES AND FUNCTIONS:

  • Oversee the duties and responsibilities of all Referral Coordinators and Referral Clerks while also performing referral coordination duties as needed.
  • Review and revise workflows as necessary to ensure maximum efficiency and effectiveness of the referral process.
  • Ensure all referrals are processed daily and in a timely manner.
  • Ensure all i2i follow-ups are completed daily.
  • Ensure all incoming calls are answered and patient messages/voicemails are returned promptly.
  • Ensure completed referrals are appropriately closed in NextGen and i2i.
  • Ensure approved authorization letters are mailed to patients in a timely manner.
  • Attend meetings related to referral coordination and provide presentations for the referrals department as needed.
  • Train and onboard new hires within the referrals department.
  • Provide ongoing training and support to Referral Coordinators.
  • Communicate updated processes, policies, and information to the referrals team.
  • Maintain ongoing communication and collaboration with clinic care teams as needed.
  • Participate in interviews, employee evaluations, and corrective actions within the referrals department.
  • Conduct daily, weekly, and monthly team huddles in person and/or via Zoom.
  • Conduct one-on-one meetings with Referral Coordinators.
  • Meet regularly with the Director of Clinics and maintain communication as needed.
  • Address and resolve patient complaints related to referrals and referral staff, both by phone and in person.
  • Process referrals and complete i2i follow-ups.
  • Review IPA portals for submitted referrals.
  • Communicate with specialist offices, providers, and patients regarding referral status and care coordination.
  • Maintain electronic logs and accurate documentation of all referrals.
  • Enter pertinent medical information into the order management section of NextGen.
  • Enter referral authorization requests into IPA and County electronic systems.
  • Obtain consultation reports and ensure documentation is properly scanned into the patient’s electronic medical record.
  • Work collaboratively with the healthcare team to ensure prompt processing of referrals.
  • Respond promptly and professionally to patient requests.
  • Treat patients, visitors, and staff with dignity, kindness, and compassion.
  • Perform other duties and responsibilities as assigned.

POSITION REQUIREMENTS AND QUALIFICATIONS:

  • Knowledge of referral procedures preferred
  • Knowledge of and experience with NextGen and EHR preferred
  • Two years of experience in a clinic setting preferred
  • Bilingual English/Spanish preferred.
  • Excellent interpersonal skills to care for patients on a continual basis
  • Excellent Communication skills both verbal and written
  • Proficient in Microsoft Word, Excel, and Data Entry
  • Able to work with minimal supervision
  • Able to work in a fast-paced environment

  • Travel to and from clinic or meeting sites within geographic service area.

DOE: $28.00 - $33.06

We are a Federally Qualified Health Center (FQHC) that provides family medicine, pediatrics, women’s health, behavioral health, dental and vision services to individuals across the East San Gabriel Valley and Pomona Communities. We practice patient-centered care and strive to serve each patient with the care that meets their individual needs.

Who We Serve

Our service area is a large, diverse, complex and rapidly changing geographic area that is home to over 700,000 people and faces significant challenges in chronic disease, including diabetes, hypertension and asthma. The majority of our patients are the working poor, and 62% are uninsured. East Valley holds public and private contracts with many funding sources, which allow us to provide medical services at very low cost to qualifying individuals. East Valley is also affiliated with two local IPAs and holds managed care contracts with most health plans operating in California. Fees for our services are on a sliding scale, but no one is turned away because of inability to pay. East Valley accepts all public health insurance, such as Medi-Cal (also known as Medicaid), Medicare, and public programs such as My Health L.A. as well as private health insurance.

Most importantly, if you or your family are not covered by any health insurance plan, our certified enrollment counselors will help you apply for Covered CA, Medi-Cal, Medicare so you can be assured your that you are accessing the best coverage options for and your family. If you don’t qualify for health insurance we can assist you with public programs, such as My Health L.A. (MHLA) in accordance to their eligibility requirements.
