Target Salary: $24.51 - $28.84 per hour

The Referral Coordinator is a key role within Northeast Arc serving as the centralized point of contact for referrals and ensuring a responsive, organized, and person-centered intake process. In this position, you will inform and guide individuals and families through available services to encourage a smooth intake process from the initial inquiry through program placement.
The Referral Coordinator is responsible for collaborating with internal program teams, supporting individuals and families with required documentation, and maintaining accurate records in the electronic medical record (EMR) system. This role also steps in to support existing staff when referrals do not meet program eligibility or when additional services are needed. Aligned with Northeast Arc's mission of empowering individuals with disabilities to live, work, and thrive in their communities, the Referral Coordinator plays a critical role in optimizing referral workflows, tracking data, and enhancing the overall experience for individuals, families, and stakeholders.
