We are looking for a mission driven Referral Care Coordinator to join our team!

Referral Care Coordinators assist patients by scheduling specialty medical appointments and services within and outside the clinic as referred by providers. Works collaboratively with the clinical team to serve as a liaison between the patient and specialty service providers to ensure patient receives quality and timely care. Ensures proper documentation in relation to outreach efforts within the database while maintaining confidential electronic health records. Maintains professional oral and written communication skills to accurately complete referral process.

This is a full-time position located on our main campus (30th & L). The schedule will be 40 hours per week; generally, Monday-Friday, 8am to 5pm.

• High school diploma and/or equivalent and basic computer skills required.
• Associate’s Degree in Health Administration and/or a related field preferred.
• Medical office and/or customer service experience preferred; 1-2 years of experience preferred.
• The ability to communicate effectively with internal and external customers.
• Demonstrated ability to: analyze and solve problems; prioritize and organize; exhibit a high level of integrity and sense of urgency to service the customer.
• Displays initiative as a team player and as a leader.
• Proficiency in English and Spanish required
