Job Description

General:

This position, under the supervision of the Director will assist the program staff with case management administrative duties and client record keeping.

Job Requirements:

  • Maintaining client records in accordance with established policies and procedures
  • Coordinating, monitoring & documenting relevant issues for clients
  • Providing technical support for program staff, including client intake
  • Serving as a liaison between the professional staff, community agencies and clients
  • Participating in all mandated staff development sessions
  • Assisting with community resource meetings
  • Facilitate transportation for clients
  • Monitor utilization of program resources
  • Contacting institutions to confirm resident departure times
  • Generating program reports to track program services
  • Tracking client participation
  • Performing other duties as assigned by Program Personnel

Qualifications/Skill Requirements:

  • One year of related experience in an office environment.

Abilities:

  • Understand and comply with company and contract agency rules, regulations, policies and procedures;
  • Communicate effectively and all levels within and outside the organization;
  • Express ideas clearly and completely, written and oral; Manage time to meet the responsibilities and duties of this position;
  • Give attention to detail and follow-through;
  • Establish and maintain appropriate working relationships with co-workers and contracting agency representatives;
  • Execute all duties in accordance with this position description.

Education requirements:

  • High School or Equivalent
