POSITION SUMMARY
The Reassessor is responsible for conducting in-home Medicaid Reassessments to determine continued eligibility and level of care for participants receiving in-home and community-based services. This position ensures compliance with Missouri Department of Health and Senior Services (DHSS) guidelines, accurately documents findings, and provides person-centered, professional interaction with clients, caregivers, and providers.
ESSENTIAL DUTIES AND RESPONSIBILITIES
The essential duties and responsibilities include the following. Other duties may be assigned.
Conduct Medicaid Reassessments in clients’ homes in accordance with DHSS requirements and agency standards.
Evaluate clients’ medical, functional, and social needs to determine continued eligibility and level of care.
Document assessments clearly and accurately in the designated software systems within required timeframes.
Verify care plan tasks and time utilization, ensuring all services align with client needs and policy.
Communicate with providers, family members, and informal supports to gather collateral information as needed.
Maintain current knowledge of program regulations, policies, and procedures.
Participate in quality assurance reviews and corrective action processes.
Adhere to confidentiality, professionalism, and ethical standards in all interactions.
Perform other duties as assigned to support the goals of the Senior Services Department.

QUALIFICATIONS
REQUIRED EDUCATION and EXPERIENCE
Bachelor’s degree in any field required.
Strong assessment, interviewing, and documentation skills.
Ability to manage time efficiently and meet strict deadlines.
Excellent written and verbal communication.
Proficiency with computers, electronic case management systems, and Microsoft Office applications.
Strong interpersonal skills and the ability to work independently and as part of a team.
Must successfully complete Medicaid Reassessment Certification
Must possess a valid driver’s license and reliable transportation for home visits.
Must pass all required background screenings.
PREFERRED EDUCATION and EXPERIENCE
Prior experience working with older adults, individuals with disabilities, or in a social services, healthcare, or case management setting preferred.
SHIFT: FT (40 hrs/week) Mon-Fri: 8am-5pm
