Job Description

We are working closely with local authority in Liverpool to assist with the appointment of a Reablement Assistant , on a 3-months contract, highly likely to be extended at clients discretion. Please apply with your CV for immediate consideration.

Rate of Pay £14.43- £18.84 per hour

The Reablement Assistant provides person-centred care and support to adults to promote independence, wellbeing, and enhanced daily living skills, enabling individuals to remain safely within their own homes and communities. Working as part of an integrated Adult Social Care and Health service, the role supports service users to meet physical, emotional, and social care needs in line with agreed care and support plans. The post holder works collaboratively with allied health professionals, social care teams, and families to minimise risks, maximise independence, and support individuals in achieving positive outcomes through reablement and recovery-focused approaches.

Responsibilities:

  • Support individuals to improve, maintain, and develop daily living skills, promoting independence and confidence in their own homes.
  • Support and participate with service users in recreational, wellbeing, and social activities to enhance quality of life and community engagement.
  • Participate in assessments, reviews, and care planning processes as required, contributing relevant observations and feedback regarding service user progress.
  • Promote safe working practices and comply with safeguarding, infection prevention, health and safety, and risk management procedures.

Qualifications:

  • GCSEs (or equivalent) in English and Mathematics, or relevant practical experience in health, care, or support settings.

Essentials

  • Valid and up-to-date classroom-based Manual Handling (Moving & Handling) certification (not online training).
  • Willingness to undertake mandatory training relevant to adult social care, safeguarding, and reablement services.
  • Experience supporting adults in social care, community care, health, domiciliary care, reablement, or support environments.
  • Experience promoting independence and assisting individuals to improve or maintain daily living skills.

Please note:

  • You should be available to work immediately or at a short notice.
  • You should have right to work in U.K
  • This role requires an Enhanced DBS
