M-F, 7:30a-4:00pm (some flexibility), no holidays or weekends
This position is responsible for collecting records, preparing and scanning records, performing quality control and validation of scanned documents. In addition, this position will include releasing medical records while complying with HIPAA requirements by following policies and procedures. These tasks require attention to detail and efficiency in producing high-quality results. This position is part of a highly motivated and compassionate team who support each other and provide great service to our customers (patients, caregivers, providers, and external facilities).
Job Responsibilities
• Retrieves records.
• Reconciles records received against those expected, follows up with patient care areas to obtain missing records; identifies issues of charts not retrieved.
• Direct and batch scan to electronic medical record system
• Performs document imaging indexing functions (Quality Control and Validation)
• Analyzes medical records to assure all required documentation is complete; if not present, assigns deficiencies to the appropriate providers, and enters and tracks deficiencies in the hospital deficiency management system.
• Applies IU Health policies and procedures, state, and federal regulations which govern the release of protected health information, including behavioral health, drug/alcohol abuse records, adoption records, and records containing information about communicable diseases.
• Provides customer service for providers and patients who need patient health information for continuity of care and other purposes; telephone courtesy and customer service skills required in addition to knowledge of how to locate and print the requested patient information from the electronic record systems as well as historical legacy system(s).
• Accurately fulfills requests, limiting the information provided to that which is needed to meet the needs of the requester.
• Maintains effective working relationships with a variety of team members within HIM as well as in other units and departments.
• Handles both verbal and written communication with team members and customers throughout the entire IU Health system.
• Maintains productivity data for all activities and submits task information daily to the productivity database
