Central Arizona Shelter Services (CASS) is seeking an experienced Rapid Re-Housing Case Manager who is passionate about helping individuals experiencing homelessness transition from shelter to permanent housing. This position works directly with shelter clients to remove barriers to housing, connect them with community resources, and provide ongoing support to help them achieve long-term housing stability.

Essential Responsibilities
- Provide intensive, client-centered case management to individuals enrolled in the Rapid Re-Housing program.
- Complete comprehensive assessments and develop individualized housing stabilization plans.
- Assist clients in locating, securing, and maintaining permanent housing.
- Build relationships with landlords and property management companies to expand housing opportunities.
- Coordinate financial assistance for move-in costs, deposits, rent, and other eligible expenses.
- Connect clients with employment services, income supports, behavioral health, medical care, transportation, and community resources.
- Maintain regular contact with clients through home visits, office appointments, and community meetings.
- Monitor client progress and adjust service plans to promote long-term housing stability.
- Accurately document services, case notes, and program outcomes within HMIS and other required systems.
- Ensure compliance with HUD, ESG, CoC, and agency policies and documentation standards.
- Collaborate with shelter staff and community partners to ensure coordinated client care.
- Participate in case conferences, team meetings, and professional development opportunities.

Requirements
Qualifications
Required
- Bachelor's degree in Social Work, Human Services, Psychology, Sociology, or a related field; equivalent experience may be considered.
- Minimum of 2 years of case management experience, with at least 1 year working in Rapid Re-Housing, homelessness services, housing programs, or a closely related field.
- Experience working with individuals experiencing homelessness, mental illness, substance use disorders, and other barriers to housing.
- Knowledge of Housing First and trauma-informed care principles.
- Experience with HMIS or comparable case management software.
- Valid Arizona driver's license with an acceptable driving record.
Preferred
- Knowledge of HUD, ESG, Continuum of Care (CoC), and Rapid Re-Housing program requirements.
- Bilingual (English/Spanish).
