RAFT is designed as a hospital-anchored, interdisciplinary mobile health program implemented through a phased hub-and-spoke structure. RAFT delivers a comprehensive, integrated service model designed explicitly for rural Vermonters whose needs are not adequately met through traditional clinic-based care alone. Phase 1 operations are anchored at Central Vermont Medical Center (CVMC), which will deploy a stable interdisciplinary core team to a defined multi-county rural catchment area. Teams operate from a mobile medical unit (the hub) supported by one to two home-visit vehicles (spokes). Focus on Medical and Preventive Care and Post-Hospital Transitions.
The Program Coordinator is primarily responsible for the successful planning and oversight of the rural access field team out of Berlin, Vermont. The Program Coordinator is a specialist in project management and advanced administrative functions. The incumbent utilizes a structured methodology to plan and implement projects; and when required, analyzes organizational systems and processes and makes recommendations for operational improvements.
This is a grant funded position.
