Essential Functions
- Accountable for the successful implementation of programs and initiatives related to quality
assessment and performance improvement to ensure achievement of Pioneers Medical
Center Excellence goals and objectives
- Build and support infrastructure to meet quality and clinical outcomes, and analytical needs
across the organization.
- Collaborate with providers and senior leadership to identify quality clinical outcomes and
patient safety concerns.
- Effect change through the development of improvement initiatives, and provide education
quality and value-based education to staff and providers.
- Create a clear and consistent approach to process improvement across all areas of the
organization including the Critical Access Hospital, Ambulatory Clinics, Clinically Integrated
Networks and the Long-Term Care Center
- Develop the stages of Care Transformation: a high reliability care blueprint involving
standardization and reducing variation, reducing medically unnecessary care, delivery of
evidence-based care, optimization of team-based care and population health programs
- Develop a framework for Care Transformation by designing models for integrating care to
meet the Quadruple Aim (improved care, decreased cost, improved patient and staff
experience).
- Implement and optimize care coordination and care transitions across the continuum,
length of stay improvement and patient throughput.
- Develop and ensure implementation of clinical and operational excellence strategy for care
delivery across the entire enterprise including defining, disseminating and monitoring key
clinical quality and value metrics.
- Provide guidance to senior leadership on the development and execution of value-based
agreements and the development of performance thresholds for incentive-based payments.

- Provide oversight of the Quality Council and related quality committees and collaboratives.
- Facilitate the organization's CMS accreditation and clinical compliance initiatives and works
with Leadership to maintain a state of survey readiness
- Lead and oversee the improvement initiatives related to evidence-based care, practicebased evidence, and care reliability.
- Lead, develop and oversee the development of centralized population health capabilities.
- Provide clinical leadership and vision in the development of a culture of quality
improvement, patient safety and clinical transformation.
- Provide expertise and leadership at all levels of the organization and acts as an organizational
leader resource related to quality, risk management, patient safety and care transformation.
- Investigate or direct the investigation of sentinel events, evaluate root causes, patterns,
or trends
- Service as the organization liaison with the CEO and legal counsel for the investigation,
processing, and defense of claims against the organization.
- Provide guidance and oversight of Ambulatory Clinics.
- Other duties as assigned.
Education and Experience
- Bachelor's Degree in Health Administration or health-related field preferred.
- Master’s Degree in Health Administration or Health-related field preferred.
- Certified Professional Healthcare Quality (CPHQ) certification preferred.
- Two to five (2-5) years’ experience in quality and/or operations improvement,
regulatory and accreditation or a combination of clinical RN experience combined with
quality experience; or project management experience required.
- Knowledge of CQI principles, practices, methods, and tools required.
- Excellent knowledge of computer processing programs, including spreadsheets.

Pioneers' history began in 1947 when Freeman Fairfield, a previous resident of this community, approached the County Commissioners about establishing a hospital. Over the next several years, Mr. Fairfield worked with the Commissioners and a group of local citizens to build a hospital for the Meeker community. Opening its doors in 1950, Pioneers began offering acute health care and surgical services to the community, licensed through the State of Colorado Department of Health and Human Services. We are Medicare and Medicaid-certified, accept private insurance, and, of course, self-pay. Financial arrangements may be set up through the Business Office.
In 1964, the Walbridge Memorial Convalescent Wing opened its doors for long-term care, made possible through the generosity of the wills of Carl and Adelaide Walbridge. Attached to the north side of the hospital, the Wing, with its 33 beds, brings skilled and intermediate nursing to residents who are unable to care for themselves. Through the Wing's rehabilitation services, exercise, and activity groups, each resident's care is individualized to meet his or her physical, emotional, nutritional, and spiritual needs. The Wing is licensed by the State of Colorado Department of Health and Human Services, and accepts Medicare, Medicaid, and private-pay residents.
First-line health care services are delivered through Meeker Family Health Center. The MFHC originated in 1990 as a rural health clinic, and its physicians, nurse practitioners, and staff continue to strive to meet our community's and visitors' healthcare needs.
