The Role

The Quality Improvement Specialist (QIS) is a professional, responsible for all of the quality activities. The Quality Improvement Specialist will be accountable for the monitoring, implementing and evaluating all processes and clinical outcomes of the program to meet Health Home Policies and Procedures. QIS will be responsible for the training of new staff, ensuring documentation adherence, monitoring monthly Core Services and outreach initiatives. This work is carried out in support of the mission and goals of Samaritan Daytop Village.

Responsibilities

What You Will Do

• Responsible for the implementation and monitoring of the organization’s quality improvement program.
• Ensures that there are processes in place to monitor and measure all activities related to performance improvement and services rendered.
• Provides monthly, quarterly and annual Performance Improvement reports, trend analysis to the Program Director.
• Provides direction and coordination of quality improvement activities demonstrating knowledge of patient care evaluation, utilizing continuous quality improvement principles and methodologies.
• Responsible for ensuring processes to monitor and evaluate the coordination of care provided by workers.
• Provides guidance for the Outreach Staff through training, performs compliance reviews, ensures regulatory requirements are met and monitors documentation to ensure that the segments are properly triggered.
• Monitors the timely completion of assignments, Health Risk Assessments and Care Plans.
• Confirms Daily Submission of entries.
• Works closely with the billing department in resolving account, claim denial, and charge capture issues.

Qualifications

Who You Will Be

• Associates Degree in Psychology, Human Services, Social Work, or any related field.
• At least Three-Five (3-5) years’ job-related experience in Process Improvement and Continuous Quality Improvement preferably within a Health Home/Care Coordination program.
• Computer literacy including proficiency in Microsoft Office Suite and EHR.
• Excellent interpersonal, communication, analytical, and organizational skills.
• Experience working directly with people from diverse racial, ethnic and socioeconomic backgrounds.
• Ability to work independently and as part of a team.
• Familiarity in developing reports.
