The Quality Coordinator I – Measurement Improvement & Data Operations supports this mission by assisting the Clinical Quality Manager in advancing organizational quality initiatives and ensuring the delivery of safe, effective, affordable, and patient-centered care.

This position focuses on quality data management, performance measurement, and care gap closure while supporting quality improvement initiatives across the organization. This position is responsible for accurate entry, extraction, analysis, and reporting of quality data, medical records, and related documentation. Through ongoing monitoring of quality metrics and performance outcomes, this role helps identify opportunities for improvement and supports VHC’s strategic quality and compliance goals.

Essential Duties and Responsibilities

Measurement Improvement & Outreach

  • Monitor UDS, HEDIS, HRSA, and payer quality measures for performance gaps
  • Generate patient lists for needed services and coordinate outreach
  • Collaborate with scheduling staff to arrange appointments
  • Conduct patient outreach and document attempts in the EHR
  • Track measure improvement progress and outcomes
  • Coordinates scheduling and logistics for mobile health units to ensure efficient service delivery

Data, Reports & EHR Extraction

  • Run routine and ad hoc quality reports from AZARA
  • Extract, validate, and reconcile EHR data for reporting
  • Identify documentation gaps and provide feedback to clinical teams
  • Maintain dashboards, spreadsheets, registries, and audit tools
  • Accurately uploads quality measure closure information and documentation into payer systems to ensure timely reporting and compliance

Projects & Submissions

  • Support HRSA, UDS, state, and payer submissions
  • Support NCQA standards and accreditation.
  • Contributes to ACO-related data collection, reporting, and care coordination.
  • Participate in chart audits, PDSA cycles, and quality improvement projects
  • Maintain documentation for compliance and audit readiness

Qualifications: Education and Experience

  • High school diploma or GED
  • 2 years of healthcare experience preferred
  • Experience in quality reporting, data analysis, or EHR systems preferred
  • Strong organizational and analytical skills
  • Proficient in Excel and data management tools

Understanding of Health Care Laws and Regulations

Must have the ability to follow HIPAA guidelines as well as an understanding of policies and procedures regarding medical records put in place by the Federal Government, Medicaid, Medicare, and WV Behavioral Health Laws put forth by the WV Department of Health and Human Resources.

Computer Literacy

Knowledge of basic computer software (MS Office – Word, Excel, and Outlook) and the willingness and ability to learn electronic medical records software.

Must be able to read and interpret medical and work environment-related documents involving patient care, program descriptions, government regulations, and VHC policy and procedures.
