Position Type: Full Time
Job Shift: DayMARC, Inc. is seeking a Full-Time Quality Assurance Facilitator. This dependable individual will prepare and attend IP meetings with DDS case managers. This individual will complete all necessary documentation and will maintain emergency fact sheets for individuals with developmental disabilities. The qualified candidate would possess strong interpersonal and communication skills, as well as good judgment. Attention to detail and strong organizational skills are a must.
Full Time position available; Monday- Friday 37.5 hrs a week

ESSENTIAL DUTIES & RESPONSIBILITIES
•    Review and audits individual service plans (ISP) data, notes, health records and incident reports for accuracy and completeness.
•    Ensures IP packets are up to date and are received from outside agencies. Confirms timely receipt of staff training signature sheets.
•    Performs various tasks for IP meetings including meeting preparation, assists in collecting data and writing IP’s and follow up meeting minutes.
•    Conduct internal audits of programs and documentation to ensure adherence to applicable federal, state and local regulations.
•    Ensure person served photographs are current, in compliance with DDS, agency and regulatory requirements.
•    Assist in reviewing and tracking incident reports
•    Coordinates with nursing and PT for completion of necessary assessments.
•    Provide training and technical assistance to staff on compliance standards, documentation practices and quality expectations.
•    Supports departments in understanding and applying person- centered planning and service delivery models.
•    Perform other duties as designated by the Quality Assurance Director

MINIMUM QUALIFICATIONS:
•    High school diploma or equivalent
•    Human services experience preferred.
•    Knowledge of Microsoft Office
•    Valid driver’s license and the ability to transport people served as needed.
•    Ability to lift 30 lbs, sit, walk, stand, and drive
•    Must be able to complete all essential job duties, minimum qualifications, and physical efforts with or without a disability.
PHYSICAL EFFORTS:
•         Frequent sitting, walking, standing, and driving.
•         Ability to stand, sit, use a telephone, and type at a computer.
•         Ability to lift and transfer up to 30 lbs. and occasional lifting of nonambulatory people.
