Job Description

Provides leadership and support for a state; covering multiple service lines and/or has a large population of people receiving services and independent contractors. Responsible for state-wide quality systems through the implementation of the Network, Operating Group, and state-specific quality improvement plans. Facilitates effective decision making and collaboration with state operations with regard to quality improvement and implementation of quality improvement measures. Promotes and manages alignment with the Network, operating group, and state-specific quality improvement plans, risk management programs, and continuous quality improvement plans to meet and exceed service excellence. Supports state operations initiatives in compliance with all applicable internal and external standards. Directs and manages assigned staff.

ESSENTIAL JOB FUNCTIONS
To perform this job successfully, an individual must be able to satisfactorily perform each essential function listed below:
• Continuous Quality Improvement
◦ Represents the state on the QI Network leadership team and participates in state staff meetings.
◦ In partnership with the state leadership team and QI Director, develops state-specific continuous quality improvement plans.
▪ System Analysis, Design and Improvement:
o. Quality Improvement:
Provides quality assurance / improvement focused technical assistance and subject matter expertise with regard to process improvement initiatives.
• Collaboration with Internal and External Partners:
Implements and maintains systems and processes for effective communication with state leadership team and QI staff to elicit feedback and disseminate information.
• Education and Training:
Partners with Operations to identify training needs and to develop resources to meet those needs.
• Performance Indicators and Outcomes
• Data Systems and Analysis:
Verifies current and accurate information in Network databases for the state.
• Outcomes:
Analyzes data and proactively identifies opportunities for improvement. Partners with state leadership and the state QI team to develop and implement strategies toward improvement.
• Risk Management
◦ Incident Management:
▪ Oversees the implementation of the incident reporting system according to The Network's Incident Management Protocol.
▪ Organizes and manages monthly trended incident reporting system data to operations for their review and action.
▪ Consults with operations to follow through on critical events and identify methods to improve safety and minimize risk to clients and employees.
◦ Investigations:
▪ Conducts complex and higher-level internal investigations.
▪ Conducts and/or reviews internal investigations for incidents involving alleged abuse and/or neglect.
▪ Implements the Network Incident Management Protocol and Mortality Review Protocol in addition to Operating Group requirements.
◦ Diligence Integration:
▪ Actively participates in merger and acquisition diligence assessments and evaluation of prospective acquisitions.
• Compliance
• Compliance with Internal and External Standards:
• Develops and oversees quality and compliance monitoring systems to measure the state’s compliance with Network standards, contractual obligations, state and federal regulations and accreditation standards.
• Supports operations to develop corrective action plans in response to adverse actions that require the Network Notification Protocol and verifies implementation of those plans.
• Network Performance Audit:
Completes Network Performance Audits
• Policy/Procedure:
Works with State leadership team to develop policies in compliance with local regulatory standards and consistent with Network policies and protocols.
• Education and Training:
Provides technical assistance and direction in the development of corrective action plans for adverse actions per Network Notification Protocol and for other issues of noncompliance.
• Customer Engagement
◦ Customer Engagement:
Collaborates with Operations leadership to determine and implement local customer engagement activities.
• Survey:
◦ Collaborates with state leadership and QI Director to design the strategy for completing the survey process in the state.
◦ Partners with state leadership and operations to develop improvement plans in response to customer engagement survey results.
o. Leadership
• Directs and manages performance of QI Specialist in tandem with Operations supervisors. Completes orientation and training for assigned QI Specialists.
• Makes recommendations on applicable employee hires, transfers, promotions, salary changes, discipline, terminations and similar actions.
SUPERVISORY RESPONSIBILITIES
• Directs and manages assigned staff including performance evaluations, scheduling, orientation, and training. Makes decisions and recommendations on employee hires, transfers, promotions, salary changes, discipline, terminations, and similar actions. Resolves employee problems within position responsibilities.one required
Minimum Knowledge and Skills required by the Job
The requirements listed below are representative of the knowledge, skill, and/or abilities required to perform the job:

Education and Experience:
• Bachelor’s degree in social service
• 3-5 years of experience in IDD (intellectual/developmental disabilities) is required.
• Some supervision experience strongly preferred.
• For Adult Day Health Operating Group, - Additional Education and Experience:
• 3-5 years of experience to support DPH regulations.
Other Requirements:
• Travel as needed
Physical Requirements:
• Sedentary work. Exerting up to 10 pounds of force occasionally and/or negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Sedentary work involves sitting most of the time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.
