UI Heart and Vascular Center is seeking a 100% Nurse Clinician to support our Pulmonary Hypertension Program.
UI Health Care is home to the most comprehensive advanced heart failure program in Iowa. The Nurse Clinician within the Advanced Heart Failure Program plays a pivotal role in providing comprehensive care and support to patients facing complex cardiac conditions. This role combines clinical expertise, patient education, and coordination of care to optimize patient outcomes and enhance their quality of life. The Nurse Clinician collaborates closely with multidisciplinary teams, including physicians, nurse practitioners, social workers, and other healthcare professionals, to develop and implement individualized care plans tailored to each patient's unique needs. Responsibilities include conducting thorough patient assessments, managing symptoms, administering treatments, and offering guidance on medication management and lifestyle modifications. Additionally, the Nurse Clinician serves as a primary point of contact for patients and their families, offering emotional support, education, and resources throughout the continuum of care. Through their dedication and compassion, Nurse Clinicians within the Advanced Heart Failure Program play a vital role in improving patient outcomes and fostering a culture of excellence in cardiac care.
