Position Title: Pueblo CMA Coordinator, Enrollment
Department: Adult Services
Reports to:  Teams Supervisor
FLSA: Non-exempt

Starting Range: $22.32- $23.18 hourly * rates reflect FY25-26 and subject to change, contact hr@tre.org to confirm rates.

General Description:
Team members needed to enroll interested people into TRE services. Coordinators will work alongside people of all ages with long term care needs and assist people to design and receive services that they need within a highly regulatory environment. Enrollment Coordinators build rapport with people, families and community providers while completing essential responsibilities around referral and service/care planning. Must be adaptable to change and have the ability to produce quality work in a variety of environments and locations against multiple deadlines. This job requires comfort with technology, adaptability to people and systems, extensive driving responsibilities and a commitment for helping those in need.

Essential Duties/Responsibilities:
- Provide excellent customer service by being responsive to needs, address concerns of individuals, return phone calls and emails within 48 hours and building positive community relationships. This includes building positive rapport with the people receiving services and their interdisciplinary team that will allow for the development and implementation of services.
- Offer and complete enrollments into services as requested for HCBS-CES/SLS/DD/CHCBS/ CHRP/EBD/CLLI/CMHS/BI and corresponding state general fund programs. This includes all regional center, foster care, incarceration, and deinstitutionalization enrollments as needed.
- Complete and follow up with service referrals to provider agencies, state agencies, and community resources as appropriate, based on preferences and availability.
- Ensure enrollments are completed within 90 days or less. If the enrollment is taking longer than 90 days, the Enrollment Coordinator must document the reason in the BUS and apprise their supervisor of the reasons why. In these situations the EC may be responsible for completing work such as the 6-month review, Continued Stay Review and Monitoring needed.
- Complete and follow up with service referrals to provider agencies, state agencies, nursing facilities and community resources as appropriate in a timely manner, based on preferences and availability.
- Ensure financial eligibility for services by communicating with the El Paso, Teller, Park, and Pueblo County Department of Human Services, and other entities such as families, Social Security Administration and Medicaid as needed.
- Ensure proper transition of files for new enrollments by completing a meeting with the Service Coordinator who will be providing long-term services.
- When needed, Monitor authorized services in the Service Plan according to Regulation.
- When needed, complete 6-month reviews, quarterly monitoring and Continued Stay Reviews.
- When needed, complete the ULTC 100.2 to determine Functional Eligibility for Long Term Care Services.
- Maintain current knowledge about and assure ongoing compliance with Rules and Regulations and related practices by attending Health Care Policy and Financing required trainings and training provided by TRE as needed.
- Coordinate with other internal departments for service coordination needs including the Quality Enhancement Team, Business office, Navigation Services, Enrollment Services, Child and Family Services, Quality Review Team, and Administration.
- Network with other professionals, community developers, and local leadership to be aware and informed of the range of options available to meet the needs of all community members.
- Complete and document case management activities according to Center for Medicaid Services, Health Care Policy and Financing and TRE protocols.
- Be available during TRE Business hours, and flex time to be available on evenings/weekends as needed to provide the above services.
- Perform driving responsibilities as required and maintain eligibility of all driving requirements. A working (personal) vehicle, valid CO drivers license (unless active Military family), and minimum insurance levels ($100,000/$300,000/$100,000 liability coverage).
- Meet with supervisor at least monthly, submit weekly and monthly paperwork/updates, and attend team meetings as requested/scheduled.
- Ensure timely processing of paperwork, deadlines are met and contribute to monthly individual and team goals.
Other Duties/Responsibilities:
- Ability to work with a variety of technology including cell phones and Surface Pros/Laptops. Willing to stay connected to co-workers electronically. Working knowledge of internet, Windows 10, Skype, e-mail, Microsoft Office programs and other data base programs.
- Participate in professional opportunities (committees, workshops, conferences, journal readings) as needed/interested/requested to foster professional development.
- Maintain master records, correspondence, documentation, and all other required consumer data.
- Participate in All-Staff and other TRE facilitated meetings and activities.
- Provide coverage for other SCs as needed and/or requested.

Qualifications:
Education and Experience Requirements:
Minimum: A bachelor's level degree of education, five (5) years of experience in the field of human services, or some combination of education and experience appropriate to the requirements of the position. (experience must be specifically documented and verifiable on your application.). Excellent written and verbal communication skills. Strong advocacy skills.
Preferred: Graduation from an accredited college or university with a bachelors degree in Sociology, Psychology, Education, Human Services, or other closely related field. At least one-year experience in a case management environment. Experience working with families and community resources; knowledge of family dynamics and resource development. Working knowledge of Medicaid Waiver programs and requirements; experience working with persons with disabilities. Knowledge of and experience working with community resources and resource development.
Material and Equipment Directly Used: Computers/tablets, Printers, Copy Machines, Personal Vehicle, Cell phones/telephone, Cloud based systems and software, etc.
Working Environment and Physical Activities Office, community, private residences, commercial properties, and virtual environment settings. Moderate physical activity required, such as walking, sitting, standing, and lifting up to 20 pounds. (Provides ADA accommodations at the request of the employee.) A working vehicle, valid CO driver’s license, (unless active Military family) is required, and auto insurance coverage with bodily injury and property damage liability coverage ($100,000/$300,000/$100,000 coverage recommended) are required.
