Public Health Social Worker

Augusta, ME

Pay Rate: $64,480.00 annually

Staffing Solutions Organization (SSO) is seeking a compassionate and resourceful Public Health Social Worker to support families across Maine through two critical public health programs within the Maine CDC:

  • Childhood Lead Poisoning Prevention Program (CLPPP)
  • Children and Youth with Special Health Care Needs (CYSHCN) Program

This unique role provides direct support to families facing challenges related to childhood lead exposure, complex medical needs, healthcare access, housing, transportation, and other social determinants of health. The successful candidate will help families navigate services, remove barriers to care, and improve health outcomes for some of Maine's most vulnerable children.

Position Highlights

  • Hybrid schedule with a minimum of two days per week in the Augusta office
  • Monday through Friday schedule, generally 8:00 AM - 5:00 PM

  • Meaningful case management and care coordination responsibilities

Childhood Lead Poisoning Prevention Program (50%)

  • Assess family needs and connect families to social services and community resources
  • Support families of children with elevated blood lead levels or lead hazards in the home
  • Assist with transportation, housing, food security, healthcare access, and other identified needs
  • Help families implement lead poisoning prevention strategies, including home lead dust testing and wet-cleaning practices
  • Assist families with lead hazard control grant applications and related paperwork
  • Participate in multidisciplinary case review and case management meetings
  • Collaborate with schools, housing providers, community organizations, and state agencies
  • Represent Maine CDC in cases involving the Office of Child and Family Services (OCFS)

Children & Youth with Special Health Care Needs Program (50%)

  • Provide care coordination and systems navigation for families of children with complex medical needs
  • Connect families with medical, behavioral health, specialty care, and community-based services
  • Support families accessing pediatric private duty nursing (PDN) services and related resources
  • Guide families through referral processes and healthcare system navigation
  • Partner with hospitals, providers, MaineCare, and community agencies to improve service coordination
  • Identify and address barriers to care, including transportation, workforce shortages, and service accessibility

Key Responsibilities

  • Conduct family needs assessments and develop service connection plans
  • Coordinate referrals to medical, social service, housing, transportation, and support programs
  • Document family interactions, referrals, and case outcomes
  • Maintain collaborative relationships with healthcare providers and community partners
  • Support equitable access to healthcare and community services
  • Participate in home visits as needed
  • Assist families in overcoming barriers affecting child health and well-being

Qualifications

Required

  • Current licensure in a healthcare or human services field such as:
  • Social Work
  • Nursing
  • Counseling
  • Related behavioral health or healthcare profession
  • Experience in case management, care coordination, or family support services
  • Experience assessing family needs and connecting individuals to appropriate resources
  • Knowledge of community services and support programs
  • Ability to work independently while collaborating across multidisciplinary teams

Preferred

  • Master's Degree in Social Work (MSW) or a closely related field
  • Experience working with children with complex medical needs
  • Knowledge of healthcare systems and care coordination practices
  • Familiarity with pediatric specialty services and home health resources
  • Experience serving diverse populations, including New Mainer communities
  • Knowledge of childhood lead poisoning prevention and environmental health issues

Work Environment & Travel

  • Hybrid work arrangement
  • Minimum of two in-office days per week at: 286 Water Street, Augusta, Maine
  • Occasional statewide travel for family meetings, home visits, and community partner engagement
  • Standard schedule: Monday through Friday, 8:00 AM - 5:00 PM, with flexibility as approved by the supervisor

Compensation for roles at Staffing Solutions Organization LLC (SSO) varies depending on a wide array of factors including, but not limited to, the specific office location, role, skill set, and level of experience. As required by applicable law, SSO provides a reasonable range of compensation for this role. In addition, SSO provides a range of benefits for this role.

Applicants must be authorized to work in the U.S. This position is not eligible for work authorization sponsorship (including H-1B sponsorship) by SSO and under the federal laws relating to the F-1 STEM EAD program, this position is not eligible to support a STEM EAD extension application.
