Scope of Position for Contract RN
SALARY RANGE: up to $40.00 per hour (excluding markup)
HOURS OF WORK: 32 to 40 hours per week; 5 days each week; 8 hours each day; 8:15 5:00 with 45 minutes for lunch
LENGTH OF ASSIGNMENT: Twelve Months could be extended
Scope of Work:
· Provides home visits for UAI program. Able to travel to clients home with or without accompany of the locality Social Worker.
· Places UAI information into the ePas Medicaid system electronic program.
· Works multiple localities within the Crater District based on need for the UAI program.
· Ensure all UAIs are completed prior to the 30 day time frame for the request date as stated by the program requirements.
KSAs
· Must have VA RN licensure or eligible to obtain and professional CPR/AED certification.
· Able to communicate effectively with clients, providers, and health department team members.
· Able to read and interpret policy regarding the LTSS Virginia program.
· Able to interpret and document accurately and effectively on the medical record.
· Ability to work with diverse cultural backgrounds needed.
· Extensive knowledge and experience in use of computers and computer applications required, skilled in internet, email and Microsoft Office.
· A valid State Drivers License is required to operate a state vehicle or if operating a personal vehicle while conducting business on behalf of the agency.
Employment is contingent upon satisfactory results of a state and federal criminal history background check and the Department of Social Services Child Abuse and Neglect Central Registry check, U.S. HHS IG Exclusion List check, employment reference check, proof of education (diploma or transcripts), professional licensure verification, and E-Verify. Conviction information, ascertained through a fingerprint-based background check, is a final step in the application process (if selected for the position).
