Job Description

DUTIES:

  • Facilitates and advocates for client access to services that provide basic needs, education, housing, health care, legal and counseling assistance.
  • Conducts home visits to assess the needs of families and provides services in collaboration with public health nurses and other service providers. Manages a caseload.
  • Develops a Family Service Plan and intervention strategies for each client and family served by the program.
  • Delivers services according to established advocacy and case management protocols.
  • Maintains accurate, legible and current documentation.
  • Meets regularly with Supervisor for consultation and evaluation of Family Service Plans.
  • Provides client teaching and training on how to access community resources.
  • Works collaboratively with clinical provider at the primary care sites through participation in case conferences.
  • Provide education, technical assistance, and advocacy for clients.
  • Implements outreach strategies to engage clients in community based services.
  • Work closely, collaboratively and in a confidential capacity with HSS staff
  • Work to help BPHC become an antiracist organization by meeting or exceeding the standards set by the BPHC Anti-Racism Policy.
  • Performs other duties as required

This job description is based on the Public Health Advocate job series. May be required to perform any and all duties and responsibilities of a Public Health Advocate I or II, as set forth in the BPHC Public Health Advocate job series.

Qualifications

  • Associates or BA/BS required or equivalent experience may substitute.
  • 2-3 years experience in health care, public health or social service field.
  • Knowledge and experience in maternal and child health.
  • Good knowledge of interviewing techniques.
  • Demonstrated cultural competence with diverse ethnic, cultural and socio-economic groups.
  • Knowledge of local community organizations, the courts, and government agencies.
  • Good organizational skills with the ability to develop timelines, meet deadlines and be self-directed.
  • Good interpersonal skills and ability to communicate effectively both verbally and in writing.
  • Commitment to health equity and racial/social justice issues.
  • Knowledge of or willingness to learn about the role of public health in addressing racism, the social determinants of health, and inequities in health outcomes as well as strategies to advance racial justice and health equity.

Preferred or Plus:

  • Bilingual fluency in Spanish preferred.
  • Experience in health care, public health or social service field preferred.
  • Home visiting or family work experience preferred.
  • Valid MA driver's license and daily access to vehicle preferred.
  • PC skills preferred
  • Experience in community outreach/case management activities a plus.
  • 1 year of demonstrated formal or informal social justice advocacy experience and/or community organizing/mobilization experience.
