Facility: Gateway Post Acute
Location: Porterville, CA
Employment Type: Part-Time
Pay Range: $40.00 – $45.00 per hour (BOE)

Gateway Post Acute is a premier skilled nursing and post-acute rehabilitation facility in Porterville, CA. We are currently seeking a compassionate and motivated Physical Therapist Assistant (PTA) to join our rehabilitation team on a PRN basis

This role is ideal for a PTA who enjoys flexible scheduling while delivering high-quality, patient-centered care in a collaborative rehab environment.

General Purpose

The Physical Therapist Assistant (PTA) assists the Physical Therapist in providing direct patient care, implementing treatment plans, documenting patient progress, and supporting residents in achieving their highest level of functional mobility and independence in accordance with physician orders, state practice acts, and facility policies.

Essential Duties and Responsibilities

  • Treat patients as directed by the supervising Physical Therapist

  • Implement treatment plans in accordance with physician orders and established goals

  • Document daily treatment notes and progress per state and facility requirements

  • Communicate patient progress, concerns, and response to treatment with the supervising PT and interdisciplinary team

  • Participate in patient care conferences, rehab meetings, and discharge planning as needed

  • Assist with training of Restorative Aides as appropriate

  • Educate patients and caregivers on exercises, mobility, and safety techniques

  • Maintain clean, organized, and safe treatment areas

  • Report equipment issues promptly to ensure proper functioning

  • Maintain positive, professional relationships with patients, families, and staff

Supervisory Responsibilities

  • This position does not have supervisory responsibilities

Qualifications

Education & Experience

  • Graduate of an accredited Physical Therapist Assistant program

  • Skilled Nursing Facility or post-acute experience preferred

Licensure & Certifications

  • Current, active California Physical Therapist Assistant license in good standing

  • Must meet all continuing education requirements per state regulations

Skills

  • Proficient in electronic documentation systems (PointClickCare and/or Casamba preferred)

  • Strong communication, organizational, and clinical skills

  • Ability to work independently and as part of a collaborative rehab team

  • Skilled Nursing Facility / Post-Acute Rehabilitation
