Under the supervision of the program director for the Transitional Living Facility the social worker will provide clinical functions which include admission and discharge functions, assessments, case management duties, consultation, implementation of treatment programs, implementations of treatment plan assignments, linkage with community resources to ensure consumers meet targeted treatment and community integration goals. Performs related duties as required. Mandatory overtime is an essential function of this position. Applicants selected for employment must complete a chemical urinalysis & breath analysis drug screening test. Applicant must successfully complete a background check. All employees are subject to drug testing for probable cause as set forth in bureau and agency policies. This is a tobacco free facility.
