Psychiatric Mental Health Nurse Practitioner (PMHNP) – Community Mental Health

  • Salary: $120,000 - $130,000 annually (Dependent on Experience)

  • Job Type: Full-time (W2)

  • Location: Salt Lake County, UT (On-site / Community-Based)

  • Schedule: Monday – Friday, Day shift with rotating on-call evening backup coverage

  • Clinical Setting: Community Mental Health / field services (Coverage area up to 500 square miles)

About the Role

We are seeking an autonomous, clinically strong Psychiatric Mental Health Nurse Practitioner (PMHNP) to serve as the medical cornerstone of a multidisciplinary community mental health team. In this role, you will provide comprehensive psychiatric evaluations, ongoing medication management, and medical consultation for adults living with serious mental illness.

Operating directly out of the community rather than in a traditional clinic or hospital setting, you will bring psychiatric expertise straight to where clients live, work, and spend their time. This role is grounded in a recovery-oriented philosophy and requires a genuine commitment to serving individuals with conditions such as schizophrenia, schizoaffective disorder, and bipolar I disorder who have historically struggled to engage with traditional outpatient services.

Key Responsibilities

Clinical & Client Services

  • Community Psychiatric Evaluations: Conduct comprehensive psychiatric evaluations for new recipients, including mental status examinations, diagnostic formulations, and risk assessments directly in the field.

  • Medication Management: Manage psychotropic medication regimens with attention to efficacy, side effects, adherence, and client preference, utilizing shared, person-centered decision-making.

  • Assertive Outreach: Conduct psychiatric visits in diverse community settings, including client homes, shelters, and other field locations.

  • Laboratory & Medical Monitoring: Provide ongoing psychiatric monitoring, including laboratory review, metabolic screening, and coordination of necessary medical referrals.

  • Crisis & Transition Coordination: Provide backup crisis consultation to on-call clinical staff and coordinate care with emergency departments and inpatient units during psychiatric emergencies. Assure seamless medication bridging during hospital admissions or discharges.

Team Collaboration & Administration

  • Daily Interdisciplinary Meetings: Participate in daily team meetings, contributing psychiatric assessment data and medical perspective to the team's shared treatment planning.

  • Clinical Consultation: Provide medical oversight and clinical consultation to all members of the team regarding psychiatric symptoms, medication management, and medical concerns.

  • Compliance & Documentation: Maintain detailed, compliant documentation of all clinical contacts, assessments, and medication decisions in the Electronic Medical Record (EMR) in accordance with county, payer, and agency standards.

Qualifications & Requirements

Education & Experience

  • Degree: Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited nursing program with a specialization in Psychiatric Mental Health Nursing.

  • Experience: Minimum of two (2) years of post-graduation clinical experience in community mental health, public-sector psychiatry, or outpatient psychiatry serving individuals with serious mental illness.

  • Preferred Skills: Experience with or willingness to engage in assertive outreach, home-based, or mobile psychiatric services. Experience with co-occurring mental health and substance use disorders, including familiarity with buprenorphine/naloxone (MAT) protocols, is a major asset.

Licenses & Certifications

  • Current, unrestricted Advanced Practice Registered Nurse (APRN) licensure with prescriptive authority in the State of Utah (Required).

  • Psychiatric Mental Health Nurse Practitioner board certification ( PMHNP-BC) through the American Nurses Credentialing Center (ANCC) (Required).

  • Active DEA Controlled Substance Registration (Required).

  • Active CPR / BLS Certification (Required).

  • Valid Driver’s License and meeting agency insurance eligibility requirements to cover a 500-square-mile service territory (Required).
