
Full-Time | Monday-Friday | 8:30 a.m. to 5:00 p.m. Occasional Evenings, Weekends, and Holidays as Needed Community-Based Field Position
This role will be based out of Central and Eastern Wake County

As a Provider Relations Representative, you'll serve as a trusted partner to physicians, hospitals, skilled nursing facilities, assisted living communities, and other healthcare organizations throughout our service area. Your work will directly support timely referrals, improve patient access to care, and strengthen relationships that positively impact thousands of patients and families.

This is not an office-based sales role.
You will spend the majority of your time in the community building and maintaining relationships with referral partners throughout your assigned territory.

Celebrating life since 1979, Transitions LifeCare was founded as Hospice of Wake County in 1979 and provides physical, emotional and spiritual care to those living with an advanced illness, their caregivers and those who have lost a loved one.
Our services include hospice care, palliative care, pediatric hospice and palliative care, caregiver support, and grief counseling. Services are provided regardless of age, race, religion or financial circumstances.
