As a Project Officer, you will work alongside health professionals, community organisations, government stakeholders and people with lived experience to deliver initiatives that support people to age well, access quality palliative care, and remain connected to their communities.
This is a hands-on role suited to someone who enjoys building relationships, coordinating projects, facilitating discussions, solving problems and turning ideas into action. You will thrive if you are organised, proactive, comfortable engaging with diverse stakeholders and passionate about improving health outcomes for the community.
This position is offered as a full-time fixed term appointment until 30 June 2028, with the possibility for extension.
As a Project Officer, you will coordinate and support the planning, implementation, monitoring and evaluation of community-based primary health care projects aligned with GCPHN's strategic priorities. Working with internal teams, health care providers, partner organisations, consumers, carers and broader community stakeholders, you will assist in coordinating project activities, supporting community engagement activities, monitoring performance measures, preparing reports and recommendations, facilitating stakeholder consultation, engagement and contributing to continuous improvement initiatives.
A key focus of this role is supporting the delivery of the Greater Choice for At Home Palliative Care Program. This includes supporting the implementation and governance of the Compassionate Communities Gold Coast model and framework, coordinating Palliative Care Education and Training, and promotion, maintenance and evaluation of Advance Care Planning resources and activities.
The role will also provide secretariat support to the Palliative and Aged Care Committee and contribute to the implementation of the CARE PATH pilot, helping to strengthen access to person-centred palliative and aged care services across the Gold Coast region.
The role requires strong organisational, communication, community engagement and relationship management skills, together with practical experience supporting community health care, primary health care, public health or related human services projects. The successful candidate will be able to work with diverse stakeholders, manage competing priorities and contribute to innovative, evidence-based solutions that improve primary health care outcomes for the Gold Coast community.
