KEYs Program and My Path Initiative

The KEYs Program is a Western Sydney Service Delivery Reform Group initiative designed to provide a more comprehensive and coordinated response to families experiencing vulnerability and disadvantage, particularly those with at least one child aged five years or younger. The program seeks to remove organisational barriers and improve collaboration across agencies to deliver care that is value-based, evidence-based and person-centred, ensuring families receive timely and integrated support.

The My Path Initiative is an innovative early intervention program coordinating the local pilot of a Violence Reduction Unit within the Nepean Police Area Command. Delivered in partnership with government, non-government and community organisations, My Path focuses on children and young people aged 8 to 14 years who are at risk of future disengagement, vulnerability or contact with the justice system.

Together, the KEYs Program and My Path Initiative demonstrate a shared commitment to early intervention, cross-sector collaboration and person-centred service delivery. While KEYs focus on supporting vulnerable families during the early childhood years, My Path targets older children and young people at risk of adverse outcomes, creating complementary pathways of support across different stages of a child's development.

What you’ll do

To be successful in this role you will have strong communication skills, an ability to establish and maintain relationships with government and non-government agencies.

The primary responsibilities will include:

  • Provide a central point of contact for the screening and assessment of young people and families, referred to the service, in accessing and coordinating service provision.
  • Work collaboratively to obtain organisational information regarding clients, to better understand individual and family needs, current service delivery and any service gaps/unmet needs.
  • Build and maintain productive relationships with government, non-government and community stakeholders and local service providers, to strengthen service integration and coordinated responses.
  • Coordinate and participate in cross-agency case discussions, local governance groups and network meetings to support collaborative planning and service delivery for vulnerable children.
  • Develop strong partnership with key service providers to understand available services, develop effective referral pathways and negotiate services which best meet family needs. This may include advocacy regarding access and service delivery.
  • Identify options for brokering external service provision where supports are not available within the current service system in line with agreed processes.
  • Monitor program activities, deliverables and outcomes, identifying risks, emerging issues and opportunities for continuous improvement.
  • Demonstrate a strong commitment to ethical and evidence-informed practice, including social justice, empowerment, cultural safety, child-centred approaches and the meaningful participation of young people and families in decision-making.

What we’re looking for

  • Demonstrated understanding and experience of the service system which supports children and their families, and the interconnectivity between government and non-government agencies involved in supporting families.
  • A confident communicator who can engage and support a diverse range of stakeholders
  • Strong engagement and relationship building skills
  • Highly resilient and able to work under pressure and manage competing priorities
  • Ability to understand and adhere to Legislation, Policies and Procedures
  • Ability to plan and prioritise workload with attention to detail
  • Current driver’s licence and willingness to travel

Appointments are subject to reference checks and the following pre-employment checks:

  • National Criminal History Record Check in accordance with the Disability Inclusion Act 2014
  • Working with Children Check clearance in accordance with the Child Protection (Working with Children) Act 2012
