Mount Sinai Health Partners (MSHP) is the team driving this transformation within Mount Sinai. The team includes 400+ employees with clinical, contracting, finance, IT, analytics, operations, product development, and community-based expertise.

Within MSHP, the Behavioral Health team is responsible for design, development, implementation, and ongoing evaluation of strategic programming that promotes efficacious and effective integration of behavioral health services across the health system. Through these efforts, MSHP’s Behavioral Health Team aims to deliver sustainable and scalable, culturally competent solutions that improve clinical outcomes, improve patient and provider experience, and lower the cost of care.

Mount Sinai Health Partners is seeking a dynamic and dedicated Manager to join the Behavioral Health team. The main goal of the Manager, Behavioral Health Integration is implementation and management of targeted behavioral health integration programs across the system. The Manager reports to the Director of Behavioral Health Population Management. They will also work closely with clinical staff at all levels of the health system, pilot primary care sites, colleagues within MSHP including pharmacy, quality, communications, care management, finance, CDQI, and analytics, and partner community-based provider organizations.

Responsibilities

Behavioral Health Referral Access Program
Oversee strategy and day to day operations of the Behavioral Health Referral Access Program in each of the following domains
MSHS Stakeholder Relationships
• Build accountable and collaborative relationships with key stakeholders within MSHP, MSHS primary and special care sites, and the MSHS department of psychiatry
• Meet regularly with primary and specialty care leaders to provide key updates and education on program goals, performance, and outcomes
• Scale Referral Access Program to additional MSHS primary and specialty care sites.
Trusted Behavioral Health Network
• Build and sustain lasting partnerships with external provider partners through the Trusted Behavioral Health Networks
• Maintain and regularly update program information, including but not limited to program database, partner services brochures, and referral mechanisms
• Regularly review Trusted Behavioral Health network partners’ performance to ensure consistent access to high quality services
• Maintain regular meetings with network partners to review performance and make improvements as necessary.
Social Work and Care Coordination Staff Support
• Regularly review performance of outreach social work and care coordination staff through aggregate data as well as documentation/chart review
• Meet regularly with outreach social workers and care coordinators to provide key updates and education on program goals, performance, and outcomes
• Provide support with resolving individual patient-level issues, as needed.
Data Review and Reporting
• Update program documentation mechanisms, as needed, to ensure clear and efficient documentation of referrals in electronic health record
• Collaborate with MSHP analytics to ensure program data is reported in a timely, thorough, and accurate manner
• Prepare program reports, as needed for MSHP and MSHS leadership
• Ensure prompt communication on any issues to ensure speedy resolution.
Program Strategy and Innovation
• Develop innovative solutions to improve program reach, visibility, and effectiveness, including but not limited to searchable web-based database, and novel referral mechanisms
• Provide strategic support on new directions for program success and reach.

Outreach for HQIP behavioral health measures
• Provide support and oversight for social work outreach to patients discharged from MSHS emergency departments for visits related to mental health or substance use.

Education and Experience
? Bachelor-s degree required (Masters preferred)
? Minimum of 5 years? experience of prior healthcare industry experience required, preferably in the areas of administration, project management, change management, health care delivery re-design or integration, health policy reform
? 3-5 years of project management experience (preferred)
? Experience managing relationships with large systems (such as healthcare organizations, state/city departments of health etc.)
? Demonstrated interest in improving healthcare outcomes, behavioral health, social determinants of health, and health equity
? Experience working within fast-paced, multi-stakeholder environments, preferred.

General Skills and Competencies
? Superior project management skills and persistent focus on key business objectives
? Very strong fluency in Microsoft Office
? Ability to rapidly create and refine PowerPoints presentations and 1-page summaries
? Results oriented with a superior work ethic and persistence
? High EQ and ability to establish strong relationships with diverse internal and external stakeholders
? Conflict resolution and management skills
? Independent worker who is organized, reliable, detail-oriented, and consistently meets timelines and objectives

? Ability to proactively and creatively address any challenges that may arise during project execution
? Effective communicator with excellent written and verbal communication skills who can convey complex ideas straightforwardly, interact effectively with different audiences and levels of seniority in a professional manner
? Ability to evaluate and demonstrate project effectiveness.
