As a Project Coordinator at Horizon, you will…

  • Become familiar with the SBIRT public health model and the multiple screening tools being used as part of this program: CRAFFT 2.1+N, PHQ-A, PHQ-9.
  • Act as a liaison between the schools (or other program settings) and HHS to implement SBIRT services with adolescent students including referral processing and scheduling.
  • Schedule and coordinate school-based trainings and other community-based meetings that take place over the course of the grant period.
  • Work with schools (or other program organizations) to communicate information about the program in emails, newsletters, and other available communications.
  • Ensure consent is obtained for referrals to school-based clinics.
  • Ensure all screening, intervention, consent, outcomes measures and other program-related documentation is entered into databases/dashboards/systems/SPARS in a timely manner and is stored properly.
  • Assist Project Director (PD) in the development, writing and submission of grant required reports and other programmatic reports as requested.
  • Make calls; perform outreach to families/providers/support services at the direction of Project Director (PD).
  • Obtain and utilize screening tools in other languages and schedule interpretation services, when needed.
  • Schedule referral appointments within reasonable lengths of time.
  • Ensure that Family Peer and Case Management services are offered to all SAFER participant parents/ guardians at the time of referral, and schedule accordingly.
  • Ensure all patient records are complete and no information or data are missing.
  • Perform outreach to at-risk youth and other student groups within the catchment area to educate/engage youth in SBIRT including brief presentations and public speaking engagements.
  • With PD, and Case Manager, assist with the advancement of operational and clinical objectives including implementation of Tickit Health wellness screenings within partnership schools.
  • With PD and Case Manager, attend community-based meetings of relevance to the program.
  • Assist to put partner agreements into place.
  • Participate in developing a sustainability plan with step-by-step actions to integrate SBIRT into daily care and treatment protocols following grant period.
  • Help troubleshoot problems as they arise over the course of the program.
  • Maintain confidentiality of information being processed/stored/accessed for the program.
  • Provide team leadership and coverage for PD when needed (facilitation of team meeting, triaging referrals, and specialized communications with referral sources).

  • Bachelor’s degree or higher in Health Services Administration or a related field required
  • Exposure to managing large-scale programs, ensuring deliverables are met, and adhering to strict deadlines while working with a team.

Location

This position will be located at 699 Hertel Ave, Buffalo NY.

Hours

This is a full-time position from Monday - Friday. One evening per week may be required. Approximate Schedule: Monday - Thursday 8:00am - 5:30pm & Fridays 8:00am - 1:00pm.

Compensation

Horizon is consistently evaluating our compensation and benefit packages to ensure we remain competitive. The range of pay for this position is $23.56 to $25.24. Starting pay may be impacted based on experience and education. Compensation adjustments are evaluated annually.

*Please note: This is a grant-funded position.

Physical demands associated with this position include:

  • Sitting for extended periods, alternating between standing and sitting
  • Mobility required with repetitive wrist, hand, and finger movements
  • Handling light duties, occasionally lifting objects up to 20 pounds
