Project Coordinator – Cognitive Care

Cabrini is seeking an experienced Project Coordinator – Cognitive Care to lead an organisation-wide program focused on improving the prevention, recognition and management of delirium, dementia, cognitive impairment and behavioural distress.

Working collaboratively with Nursing, Allied Health, Medical, Patient Safety and Education teams, you will translate evidence into practice and embed sustainable clinical systems, education and governance that improve outcomes for patients, families and staff.

This is a part-time 12 months fixed term role.

About the role

This is a unique opportunity to combine your clinical expertise, leadership and quality improvement skills to drive meaningful change across Cabrini.

You will:

• Lead initiatives to improve the assessment and management of cognitive impairment, delirium and dementia.

• Provide expert clinical advice to teams managing complex behavioural presentations.

• Develop and implement screening, assessment and behavioural support pathways, including Top 5 and Safe Wards.

• Support evidence-based, person-centred and proactive approaches to care.

• Lead initiatives to reduce occupational violence and aggression (OVA) associated with cognitive impairment.

• Support the implementation of Age Friendly Health System principles.

• Partner with consumers and carers to improve models of care.

• Support education, clinical governance and sustainable practice change.

About you

You will be an experienced Registered Nurse and clinical leader with a passion for improving care for people with cognitive impairment.

You will have:

• Current AHPRA registration as a Registered Nurse.

• Minimum five years' post-registration clinical experience.

• Senior clinical leadership experience in acute healthcare.

• Experience in delirium, dementia, cognitive impairment and/or behavioural distress.

• Strong change management, collaboration and stakeholder engagement skills.

• A commitment to evidence-based, person-centred care and quality improvement.

• Postgraduate qualifications in Gerontology, Dementia Care, Clinical Leadership, Quality or a related discipline will be highly regarded.
