Job Location Bronx, NY 10456 Position Type Full Time Education Level Graduate Degree Salary Range $85,000.00 - $91,800.00 Salary/year Travel Percentage None Job Shift Day Job Category Nonprofit - Social Services Comunilife, Inc is looking for an experienced Program Director for its Bronx congregate site, El Borinquen The Program Director is responsible for program management and the quantity and quality of services provided to clients at the El Borinquen Residence; adherence to budgets and contract deliverables; oversight of outcome measurements and metrics; relationships with other providers and with government, and other funders; collaborating with Human Resources on employee hiring, discipline, evaluation, training, development, and supervision. This position is eligible for Comunilife's generous benefits and paid time off package
Essential Duties:

  • Oversee case management activities including education about independent living skills and safety, facilitation of clients’ health and rehabilitation through direct services, advocacy and referrals for mental health, medical and substance abuse services, along with psychosocial, educational, vocational services
  • Direct and supervise staff in health, mental health and substance abuse activities, lead team meetings, conduct individual supervision and provide exemplary leadership and program management; oversee the hiring process for relevant positions
  • Coordinate with other community services such as inpatient, outpatient, emergency, day treatment, Health Homes and others to ensure comprehensive access to services
  • Ensure accurate reporting and efficient and effective staff practices; develop and maintain policies, principles, and procedures that are in compliance with NYC-DOHMH, NYSOMH, Medicaid, HRA, and HUD and other relevant state, city, and borough-based agencies, programs and services
  • Build effective relationships with community partners, public officials, etc.
  • Maintain Quality Improvement (Q/I) Program activities, ensure timely and accurate documentation; track and monitor required weekly contacts; collaboration and coordination of health, mental health and substance abuse services; environmental inspections and repairs, etc.
  • Overall management and maintenance of physical facility including permits, OSHA, fire drills, inventories and other related tasks
  • Prepare reports and correspondence as requested by the Federal, State, City or other funding sources



Qualifications


Education and Experience Requirement(s) MSW from an accredited school of Social Work or related degree. New York State Certification (CSW). At least 6 years of progressive experience in the administration of Mental Health Housing programs.

Our Medical Respite program provides short-term supportive housing and case management services to multiply-challenged patients leaving hospital care without a viable home in which to recuperate.

Comunilife is a leader in the field of Latina adolescent suicide prevention. Our Life is Precious™ Program, which opened in 2008, provides non-clinical services to girls’ ages 12 to 17 in the Bronx, Brooklyn, Queens, Manhattan, Poughkeepsie, Yonkers, Hempstead, and Amsterdam. Activities include academic support, creative arts therapies, music lessons, wellness activities and concrete family services, are designed to reduce the risk factors associated with Latina adolescent suicide.
