Job Description

Compensation Range: $18/hr - $21/hr commensurate with experience

Overview:

Program Coordinator is responsible for all service logistics, supervision, and work plans for the Grant for the Benefits for Homeless Individuals (SAMHSA) project led by Intercambios Puerto Rico.

Responsibilities:

  • Coordinate all program services to individuals (and families) who are experiencing homelessness, substance use, and/or co-occurring mental health disorders.
  • Supervise personnel about task compliance, attendance, and performance.
  • Develop, implement, and monitor corrective action plans.
  • Coordinates the monthly work plan.
  • Conduct weekly brief meetings to direct the work of the team.
  • Ensuring that evidence-based practices are properly addressed and documented.
  • Coordinate and participate in case discussions. Provide recommendations and follow-up if necessary.
  • Facilitate coordination and collaboration with other community-based organizations and partners.
  • Ensure program integration with other agency services.
  • Develop a steering committee and coordinate quarterly meetings.
  • Assist in the coordination of all data gathering, analysis, and program reporting.
  • Prepare SAMHSA Progress Reports.
  • Collect monthly reports from the team, monitor service delivery trends, identify areas for improvement and make a corrective action plan.
  • Oversee data entry into the system and monitor project progress.
  • Represent organization in different forums.
  • Support the work team in everything necessary for the continuity of services.
  • Support the evaluator in the collection of information.
  • Report project progress or challenges to the project director.

Requirements:

  • BA or master’s degree in related field
  • Experience with direction or coordination of programs directed to people that have experienced homelessness, substance use, and/or co-occurring disorders in community settings.
  • Experience with supervision and disciplinary management.
  • Experience with Evidence Based Practices / Interventions
  • Knowledge about MAT/HIV/AIDS, Hepatitis-C, and STDs in the Puerto Rican context
  • Able to work closely with others in outreach team and office setting (team player)
  • Proficient and efficient with multiple computer software programs
  • Meticulous at maintaining records and confidentiality protocols
  • Outreach experience
  • Up to date driver’s license
