JOB SUMMARY

Reporting to the Program Manager of Addiction Services, the Addiction Services Counselor III is responsible for providing confidential counseling, education, and support to individuals and families experiencing substance use disorders, and for promoting health lifestyles and healthy choices to individuals, families, and the community. Addiction services are a key part of The Centers mission to provide integrated care which also includes behavioral healthcare, primary care, pharmacy and wellness services. Our spectrum of Substance Use and Addiction Services ranges from lowering risks associated with substance use, Treatment ranging from ASAM Levels 1.0 to 3.7 and close integration with our team providing Medication for Addiction Treatment.

ESSENTIAL JOB DUTIES AND RESPONSIBILITIES

• Perform Assessments for substance use disorders and co-occurring mental health disorders.
• Work collaboratively with clients to develop treatment plans.
• Provide individual, group counseling, crisis intervention, and case management services as outlined in the treatment plan.
• Provide counseling to pregnant women about the effects of drugs and alcohol on the fetus, as requested.
• Provide family counseling and/or education to help support client recovery.
• Match counseling techniques to client needs and readiness to change.
• Utilize evidence-based approaches to care, including motivational interviewing, trauma informed care, and strength-based therapy.
• Document accurately and completely all care provided using appropriate records within the allotted time frame.
• Actively monitor outcomes and make real time adjustments to ensure the program meets targets.
• Work collaboratively with behavioral health, physical health and pharmacy staff to ensure integrated care is available to all clients and provide referrals, as clinically indicated.
• Actively work with clients on discharge planning to facilitate transition to community or appropriate level of care.
• Ensure that legislation, standards, and guidelines are being followed, including but not limited to confidentiality guidelines.
• Provide community education and support programs as requested.
• Provide liaison functions with internal resources as well as community agencies and supports.
• Perform individual and group therapy sessions, case management, and referrals as outlined in the treatment plan.
• Develop and facilitate communication and education plans to help increase substance use disorder awareness among BH staff.
• Other duties as assigned.

ESSENTIAL JOB QUALIFICATIONS AND SKILLS

Requires a license through Ohio board of Counselor, Social Worker and Marriage & Family Therapist (CSWMFT) or Ohio Chemical Dependency Professional’s Board LCDC III, LSW, LPC or higher), with the appropriate level education required by the licensing board for the issued licensure and 3+ years’ experience in an SUD treatment setting.

• Maintain current state licensure/certification to function as an approved Addictions practitioner.
• Bilingual in Spanish/English preferred but not required.
• Demonstrate the knowledge and skills necessary to communicate patient and family needs and progress with other disciplines and agencies as necessary.
• Must possess a high degree of initiative, orientation toward process improvement, and demonstrate leadership qualities.
• Must have strong organizational skills, strong critical thinking, must be solution-focused, flexible, and able to multitask in a fast-paced environment.
• Must possess computer proficiency and proficiency working in an EMR.
• Must understand and embrace a systems approach to service delivery, as well as organizational mission, program philosophy, goals, and organizational values.
• Must have an awareness of and sensitivity to the cultural diversity of the population served.
• Must possess a high degree of professionalism, have strong communication, be assertive, and have the ability to engage clients and staff in building positive working alliances.
• Must be a team player, able to work autonomously, be creative, assertive, resourceful, and self-directed.
• Must have experience in monitoring outcomes, data, and performance indicators.
