Location: Remote; requiring regular travel for onsite go-lives, department rounding and other support activities as needed.

Role Scope:

We are hiring a Product Manager, Strategic Technology Applications to own the product strategy, roadmap, and cross-functional delivery for the technology application portfolio that powers Habitat Health's growing PACE organization. This is a high-impact, cross-functional role sitting within the Strategic Technology Applications team, reporting directly to the Vice President of Product.

As Habitat Health expands into new markets and scales its center operations, the Strategic Technology Applications team functions as the internal “product organization” for the systems that power clinical, operational, growth, and administrative workflows. This role will own the end-to-end product lifecycle — discovery, roadmap prioritization, delivery, adoption, and iteration — for a portfolio of applications spanning call/communications, CRM, core operations, growth, and risk adjustment systems.

The ideal candidate is a product-minded healthcare technology leader who is energized by translating clinical and operational needs into a compelling roadmap, partners naturally with internal developers and vendor teams to ship solutions, and uses data and stakeholder feedback to continuously improve adoption and impact.

Core Responsibilities & Expectations for the Role

Product Strategy & Roadmap

  • Own the product vision, strategy, and roadmap for a portfolio of strategic technology applications, including but not limited to call/communications platforms (TalkDesk), CRM platform (Salesforce), EHR platform (Epic)
  • Translate operational, clinical, and growth stakeholder needs into a prioritized backlog and roadmap, balancing quick wins against longer-term platform investments
  • Define and track success metrics — adoption, utilization, satisfaction, and operational impact — to measure product performance and guide iteration
  • Maintain a clear view of the application portfolio's health, dependencies, and integration points (e.g., Salesforce and ancillary clinical/communications tools) to ensure data integrity and a cohesive experience across platforms

Discovery, Delivery & Execution

  • Run discovery with clinical, operational, and growth stakeholders to surface pain points, validate requirements, and shape well-scoped product requirements and user stories
  • Partner with internal analysts (e.g., CRM Admin, EHR Support Analyst, etc.) and external vendors to scope, prioritize, and deliver configuration changes, integrations, and new features
  • Own backlog grooming, sprint/release planning, and prioritization decisions using tools like Jira
  • Lead or co-lead technology application implementations for new PACE center launches, including configuration, workflow validation, user acceptance testing, and go-live support
  • Build repeatable implementation and product-launch playbooks that support Habitat Health's continued growth

Vendor & Stakeholder Management

  • Own day-to-day vendor relationships for managed applications, including performance management, escalations, contract oversight support, and roadmap alignment
  • Evaluate and recommend new technology solutions in partnership with the Director of Strategic Technology Applications; support build-vs-buy assessments and vendor selection
  • Serve as a trusted product partner for clinical, operational, and growth teams, translating their needs into the roadmap and communicating trade-offs clearly
  • Represent the Strategic Technology Applications product portfolio in cross-functional forums, governance bodies, and project teams

Adoption, Training & Change Management

  • Partner with training and enablement resources to design rollout, onboarding, and training plans for managed applications across clinical, operational, and administrative end users
  • Track adoption metrics and gather user feedback; feed insights back into the roadmap and configuration priorities
  • Own change management planning for major releases, feature rollouts, and workflow redesigns
  • Maintain clear product documentation, including roadmaps, requirements, release notes, and user-facing support materials

Required Qualifications and Competencies

  • 5+ years of experience in product management, with hands-on ownership of the roadmap and lifecycle for enterprise or B2B SaaS applications; healthcare or health IT experience strongly preferred
  • Demonstrated experience managing or launching SaaS/cloud-based applications (experience with call center platforms, CRM, or clinical ancillary systems a plus)
  • Strong command of product management fundamentals: discovery, prioritization frameworks, roadmapping, backlog management, and agile delivery (Jira or similar)
  • Proven ability to translate business and clinical requirements into clear product requirements, user stories, and workflow improvements
  • Comfortable partnering with both technical teams (developers, vendors) and non-technical clinical/operational stakeholders
  • Strong written and verbal communication skills, including the ability to produce clear product documentation, roadmaps, and executive-facing updates
  • Experience supporting multi-site healthcare operations or rapid program expansion preferred
  • Familiarity with PACE, Medicare/Medicaid, or other regulated healthcare environments preferred

Nice to Have

  • Experience with TalkDesk, Salesforce, Abridge, Epic or other contact center / CRM / ambient AI documentation /EHR systems
  • Experience with health plan systems (claims, eligibility, encounter submissions) or risk adjustment platforms
  • Background in product delivery or change management methodology

Compensation:

The anticipated base salary range for this position is $170,000–$194,000 for candidates located in San Francisco, New York City, or Seattle, and $149,000–$185,000 for candidates located elsewhere. This position is also eligible for a target annual bonus of 10% of the base salary and may be eligible for additional incentive compensation, as applicable.

The final compensation offered will be determined based on factors including, but not limited to, the candidate's qualifications, skills, experience, education, certifications, geographical location, and business needs.
