As we continue to grow, we are seeking a dependable and compassionate PRN Registered Nurse (RN) to join our team and assist with in-home assessments and reassessments. This is a rewarding opportunity to make a meaningful impact while working with a supportive and mission-driven organization.
This flexible PRN role is ideal for an RN looking for supplemental hours while making a meaningful impact in the community. The nurse will complete in-home visits, client assessments, and reassessments while collaborating with our care team to ensure exceptional client support and continuity of care.
Responsibilities
- Conduct in-home client assessments and reassessments
- Complete nursing visits and required documentation accurately and timely
- Observe and report changes in client condition or care needs
- Collaborate with office staff and caregivers to support quality care plans
- Provide compassionate, professional communication with clients and families
- Assist with additional nursing support as needed
Schedule
- PRN / Part-Time
- Estimated 2-12 hours per week with opportunity to grow
- Flexible scheduling based on client needs and availability
Qualifications
- Active RN license required
- Previous home health, hospice, or healthcare experience preferred
- Experience working with Medicaid clients is a plus
- Strong bedside manner and communication skills
- Tech-savvy and comfortable using digital documentation systems
- Self-motivated with a flexible, “go-getter” attitude
- Reliable transportation for local visits
