
Referral RN - Palliative Care
PRN - as needed/variance in schedule
The Palliative Care Referral RN will work with the Palliative Care team to support clinical staff and physicians in the management of the continuum of care for palliative care patients. This includes assisting with patient evaluation, level of care determination, patient management, and goals of care conversations. The RN will assist with the education of staff and physicians. Including assisting with completing advance directives and EMS DNR forms.
Requirements:
Associate's degree in nursing required.
Active RN license through the Kentucky Board of Nursing required.
Two years of acute care experience required.
Must obtain CHPN within 3 years of hire.
Preferred Qualifications:
Bachelor's degree in nursing (BSN) preferred.
One year of hospice/palliative care experience preferred.
