Mercy Hospital’s 4A Transitional Care and Discharge Unit is a 12-bed unit that will be opening permanently to reduce Emergency Department Holding and to promote better throughput. Patients in this unit consist of those who are transitioning from acute care to home or another care setting and patients who no longer require the intensity of
acute hospital care but still need specialized medica, nursing or other care before discharge. As an LPN, you willassistfellow caregivers in providing exceptional, top-quality care to a diverse patient population. A PRN caregiver in this position works varying days from 7:00AM – 7:30PM
A caregiver who excels in this role will:
Collect, document and report patient data and observations.
Implement care plans as directed by an RN and contribute to care plan revisions.
Administer medications and/or treatments authorized by a provider andobservepatients for adverse reactions
Perform routine laboratory tests.
Educate patients and family members.
Minimum qualifications for the ideal future caregiver include:
Graduate of an accredited Practical or Vocational Nursing Program
Current Ohio license as a Licensed Practical Nurse
Basic Life Support (BLS) certification through the American Heart Association (AHA) or American Red Cross
Internal candidates:one year of related nursing experience
External candidates:two years of recent related nursing experience
Physical Requirements:
Requires full body motion to move and lift patients, manual finger dexterity with good hand-eye coordination, involves extensive standing walking and occasional lifting.
Requires corrected vision and hearing to normal range.
Requires working under stressful conditions or working irregular hours.
Requires some exposure to communicable diseases or body fluids.
Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.
Physical Demand requirements are in excess of those for Light Work.
Personal Protective Equipment:
Follows standard precautions using personal protective equipment as required.
