Job Description

Founded as Boulder Hospice in 1976, TRU Community Care is a nonprofit healthcare organization serving Boulder, Denver, and surrounding counties. With a mission to affirm life at every step of the journey with illness and loss, TRU provides a continuum of care through TRU Hospice, TRU PACE (Program of All-Inclusive Care for the Elderly), TRU Palliative Care, TRU Grief Services, and Landmark Memory Care.

Title: Hospice Phone Triage RN
Pay: $42.34 to $50.81 per hour
Schedule: PRN, minimum 2 shifts per month. Typically 12 hours each.
Location: Mostly remote, must reside within Colorado, Florida, or Georgia. Will require occasional training near, and there will be hiring preference for, the Lafayette, CO area.

TRU Community Care is looking for a phone triage nurse. This nurse will work with our afterhours and triage team, taking in calls from patients and collaborating with visit nurses on patient needs as necessary.

ESSENTIAL DUTIES
• The Triage Registered Nurse performs in a supportive role to the Care Team by answering calls coming into the agency, with response to the telephone call by delegating tasks.
• Monitors and responds to triage calls within the agency specific time limits.
• Evaluates and assesses patients’ health care needs based on patient’s signs and symptoms utilizing established triage protocols and guidelines.
• Obtains appropriate input and direction from the provider as needed to determine the best clinical course of action and recommendations for specific patient health care needs.
• May direct them to emergency room, urgent care, provide home care advice, or arrange for an in-home nursing visit.
• Utilize triage protocols appropriately.

EDUCATION & REQUIREMENTS
• Must have a secure HIPAA-compliant workspace with a door, no background noise or interruptions, and a reliable wired high-speed internet connection with a minimum of 150Mpbs download and 100Mbps upload capabilities.
• Must possess a current RN license in the state of Colorado or compact nursing license
• Must reside in Colorado, Florida, or Georgia
• Must have a minimum of one year RN experience, with hospice, home health, or PACE experience preferred
• Must successfully pass agency required background checks and drug screening
• Must comply with agency health screening and immunization requirements
