Your Role

• 
  Provide comprehensive primary
  care to older adult patients
• 
  Take ownership of outcomes while
  delivering personalized, relationship-driven care
• 
  Serve as a
  health coach
  ,
  guiding patients toward sustainable lifestyle changes
• 
  Coordinate specialty and hospital
  care while maintaining continuity
• 
  Lead and collaborate with your
  care team
• 
  Supervise and support Advanced
  Practice Providers (APPs)
• 
  Partner with leadership on
  quality, experience, and cost goals

What You Bring

• 
  MD or DO (Board
  Eligible/Certified in Internal Medicine, Family Medicine, or Geriatrics)

  Active (or eligible) Virginia

Compensation & Benefits

  $250,000 – $306,714 base salary
