We are seeking a Primary Care Physician (MD/DO) for our mobile physician group who wants to practice thoughtful, value-based care medicine. This group visits Assisted Living Facilities throughout Central Florida.

This is an opportunity to provide professional physician services in an integrated specialty and primary care organization, focused on chronic disease management, prevention, and long-term patient outcomes.

About the Role:

  • Monday – Friday, 8am – 5pm, no nights and weekends.

Upperline Health providers coordinate patients’ care among a team of specialists – Podiatric Physicians and Surgeons, Primary Care Physicians, Endocrinologists, Advanced Practice Providers, Care Navigators, Pharmacists, Dieticians, and Social Workers for integrated treatment that addresses patients’ immediate and long-term health needs. We put patients at the center of value-based care. This is a unique role designed for physicians who want more than traditional primary care.

What you’ll do:

  • Provide comprehensive primary care to a defined patient population, many with chronic conditions such as diabetes, vascular disease and mobility limitations.
  • Diagnose conditions, develop treatment plans, and prescribe medications and therapies as appropriate, working with Upperline Health specialists and care teams to proactively manage conditions, reduce complications, and improve quality of life.
  • Conduct patient assessments through medical history, physical exams, and diagnostic evaluation.
  • Order, review, and interpret laboratory tests and diagnostic imaging.
  • Coordinate and manage referrals to specialists and follow up on patient outcomes.
  • Educate patients and families on diagnoses, treatment options, medications, and preventive health strategies.
  • Maintain timely, accurate clinical documentation in the electronic health record.
  • Comply with clinical policies, quality standards, and regulatory requirements.

Experience and Qualifications

  • Medical Doctor or Doctor of Osteopathic Medicine required
  • Successful completion of accredited Internal Medicine or Family Medicine residency program required
  • Board certification or board eligibility required
  • Medical license required – active license preferred or the ability to obtain within applicable timeframe
  • Possession of DEA registration or eligibility required
  • BLS certification and maintenance
  • Open to experienced or new graduate providers
